    It will come as no surprise to anyone here that cost of living is the biggest issue for people in my electorate of Dickson, paying the bills has been a real struggle. Labor went to the election with a really great plan to address cost‑of‑living issues and part of that was supporting wage increases.

    I was really, really pleased to see the figures this week that showed 18 months of real wage growth, and that’s all down to 3 years of really hard work by Jim and his team.

    We know that under the Coalition that wages were falling and that people were going backwards. So it’s really great to have the Treasurer here today in Dickson, as well as all of my other Queensland colleagues, and I’m now going to hand over to Senator Chisholm.

    Anthony Chisholm:

    Thanks Ali, it’s great to be with you, and the growing Northside Labor team in Emma and Corrine, and we welcome Jim from the Southside to the Northside.

    Ali, Emma, Corrine and myself are all based on this side of town and in outer suburbia. We understand that the Petrie and Dickson electorates and those on this side of town are full of people who work hard every day, want to provide for their families and get ahead in life.

    A defining feature of the Albanese government in the first term has been support for wage increases. We saw it during the 2022 campaign, and we saw it during the 2025 campaign as well, and I think it was a defining element to us receiving a good vote like we did here in Dickson and Petrie to help us win these seats to be part of an Albanese Labor government.

    So I’m really pleased that the Treasurer is here today to talk to us but also outline the role the government is going to play supporting those people on award wages to get ahead in life. They work hard, they deserve a decent pay as a result of that, and it’s important that the Albanese government supports them in that endeavour as well. So thanks, Treasurer.

    Jim Chalmers:

    Thanks very much, Chis, and it’s great to be here in Strathpine with really important parts of our much bigger, much better Queensland team now in the Albanese Labor government.

    I wanted to thank and congratulate Ali France on her stunning victory here in Dickson – similarly, Emma Comer in Petrie, we’re really looking forward to working with Corrine Mulholland when she joins the Senate in July, and I also congratulate Anthony Chisholm for being sworn in as a frontbencher in the Albanese Labor government as well.

    Wages and the cost of living were front and centre in our first term, they were front and centre in the campaign, and they will be front and centre in the second term as well.

    Decent pay, better wages, decent conditions, great jobs, these are Labor’s reasons for being, and you can see that in the progress that we’ve made together on wages, on jobs, in the labour market and the economy more broadly, and you can see it in the submission that we are lodging today.

    Today we are lodging our submission to the Fair Work Commission’s Annual Wage Review, which is all about recognising that millions of Australians on awards need and deserve decent pay so they can work hard and provide for their loved ones.

    The most important feature of today’s submission is we are seeking an economically sustainable real wage increase for Australians on awards.

    This is all about ensuring that 3 million Australians can get the decent pay that they need and deserve to provide for their loved ones.

    We’re very proud to be making this submission today, because it builds on the progress that we have made together when it comes to wages and jobs.

    This submission is responsible, it is fair, and it’s consistent with our efforts to provide tax cuts for every Australian taxpayer as well.

    This Albanese government is all about ensuring that Australians earn more and keep more of what they earn, and our submission today to the Fair Work Commission reflects that objective.

    It does build substantially on the very encouraging progress that we have been able to make together on wages and in the labour market more broadly.

    Already, people on the minimum wage are earning $143 a week more since Labor came to office. Australians on the medium wage are earning $206 a week more since Labor came to office. We’ve created 1.1 million jobs since we were elected. Participation is at or near record highs. Average unemployment has been historically low.

    Just this week, as Ali said, we got very encouraging news on wages, 18 consecutive months of annual real wages growth, the strongest real wages growth for 5 years. Another 89,000 jobs created in the data that we saw just yesterday. This shows we have been making progress together, and the submission we lodged today is about building on that progress.

    If you look more broadly across the economy since we came to office, real wages were falling sharply when we came to office, we’ve turned that around, but we’ve made progress more broadly on the economy as well.

    Inflation is down very substantially, real wages are up, unemployment is very low, growth is rebounding in our economy, we’ve got the debt down, interest rates have started to come down earlier in the year as well.

    We know that there’s more work to do because people are under pressure, and that’s why this submission today seeks a real wage increase for millions of Australians. We have made a lot of progress together, and we seek with this submission today to build on that progress so that Australians can earn more and keep more of what they earn, and so Australians can earn more to provide for their loved ones when they work hard and get ahead.

    Happy to take a few questions.

    Journalist:

    What do you mean by ‘economically sustainable amount’, is that in line with inflation, or is there a figure on that?

    Chalmers:

    Consistent with our earlier submissions, we don’t put a number in our submission, that’s been our practice for really quite a while now. What we are seeking is an economically sustainable real wage increase for millions of Australians on awards – and ‘economically sustainable’ reflects the fact, and you can see that in the detail of our submission, is that we want to make sure that this real wage increase is provided consistent with our other economic objectives, by getting inflation down and our other economic objectives as well.

    We’re obviously very focused on the fight against inflation, we have made a lot of progress there, but it’s not mission accomplished because people are still under pressure.

    I consulted with the Reserve Bank Governor as we finalised this submission. The Treasury also consulted with, I think, the Assistant Governor of the Reserve Bank to make sure that what we are proposing is responsible, it’s sensible, it’s sustainable, and it’s consistent with inflation being sustainably in the Reserve Bank’s target band, and I’m really confident that it is.

    Journalist:

    How will you avoid a budget black hole if your super tax goes through and people take capital offshore?

    Chalmers:

    A couple of things about that. What we’re proposing here is still very concessional treatment for Australians with very big superannuation balances, so we’re taking the current concessional treatment and making it slightly less concessional, but still concessional.

    This is a very modest change to the taxation of very large superannuation balances. It reflects about half a per cent of people. We announced it more than 2 years ago, we’ve done a bunch of consultation on it, it’s been in the Parliament for a big chunk of that time, and it means that there is still concessional tax treatment for people with big balances, but slightly less concessional.

    This is an important part of our efforts to make the budget more sustainable, and to fund our priorities, including strengthening Medicare, providing cost‑of‑living relief, the tax cuts for every Australian taxpayer. It’s responsible, it is modest, it only applies to a tiny sliver of people in superannuation, and it’s still concessional.

    Journalist:

    Why won’t you index, just with that indexation, start modest, and then creep up, and become [indistinct]?

    Chalmers:

    This is consistent with the treatment in a whole range of areas in the tax system. There are a lot of thresholds in the tax system and more broadly that aren’t indexed, and what that means is that governments of either political persuasion into the future can take decisions to lift the threshold; we’ve seen that, as I’ve said, in other parts of the tax system.

    Some of this analysis that you see about the thresholds in 30 or 40 years’ time, that assumes, I think wrongly, that no government of either persuasion would change that threshold.

    Journalist:

    Treasurer, can you –

    Chalmers:

    We’ll just go here and then to you.

    Journalist:

    On childcare, should taxpayers pay for these pay rises or parents out of pocket, and is that fair?

    Chalmers:

    We’ve provided billions of dollars to make sure that the early childhood educators who are doing such an incredible job for young people and for families in our communities, that they get the pay that they need and deserve.

    I was very proud to work very closely with Anne Aly and Jason Clare, and the Prime Minister and others in the course of the last term to make room for the Commonwealth contribution to these pay rises.

    This is an area with a lot of young families, so is the area that Emma represents, the area that I represent, and we know how important early childhood educators are. We want to make sure that they’re paid properly, we’ve made room in the Budget for billions of dollars to make sure that that’s a reality.

    Journalist:

    Treasurer, can you explain how defined benefits pensions will be taxed? How’s it calculated, what’s in [indistinct].

    Chalmers:

    The actuarial calculation is similar to the calculation that currently applies to the changes that the Coalition made when they were in office. There’s a formula which is calculated by actuaries and applied by the Tax Office in a way that is not inconsistent with the way it’s currently calculated to some of the changes that my predecessors made.

    Journalist:

    Treasurer, what is your reaction to Gerry Harvey saying a tax on unrealised capital gains is gross stupidity of the highest order?

    Chalmers:

    It’s not unusual for him to criticise Labor governments. I try and listen respectfully when people make a contribution to the national public policy conversation, but I think in Gerry’s case, he’s a relatively frequent critic of Labor governments. I don’t get too carried away by it, nor do I dismiss it.

    If you look at some of the commentary over the last couple of days, you know, there was one piece that was pretending that Campbell Newman, of all people, was some kind of observer of Labor government policy.

    You had one Liberal politician, whose primary purpose was to raise campaign donations, you had another Liberal politician lie about there being no legislation available when he was on the Committee that scrutinised that legislation in detail.

    I understand that when you’re making a change, even a modest one like this one, people have views about it, and people with very large superannuation balances will have views about it, political opponents will have views about it as well.

    This is a modest change, it makes a meaningful difference to the budget, but it still provides very concessional treatment for people with more than $3 million in superannuation, and it helps make the budget more sustainable and fund our priorities.

    Journalist:

    The vaccination rates among children and teenagers have dropped to critical levels across the country. Will the government put more money into urgent campaigns or other awareness campaigns to encourage parents to get their kids vaccinated?

    Chalmers:

    I’m sure that that’s something that Mark Butler, the Health Minister, is considering, but we already put a lot of effort into educating and encouraging people to get vaccinated.

    I personally found that story to be quite confronting to think that after all of the progress that’s been made in recent decades that we’re going backwards, I personally find that very troubling, very concerning and very confronting, and I’m sure the Health Minister’s in the same boat, and he’s working out what, if anything, else we could do to try and arrest that slide.

    Journalist:

    The $150 electricity rebate’s due to run out at the end of the year. Is the government open to considering extending that, considering the affordability crisis?

    Chalmers:

    Well, we’ve already extended those electricity bill rebates, that’s the $150 you refer to in your question. They were otherwise due to run out at the end of next month, and now they’ll be extended for another 6 months.

    From budget to budget, we evaluate the circumstances we’re in, we look at the pressures on people and the pressures on the Budget as well, and we do what we can to help out. That’s why, and my colleagues here would know this, having spent so much time engaging with people in their own communities, the highest priority of the Labor government in the first time was to get on top of inflation and help people with the cost of living.

    Electricity bill rebates are an important port of that, 3 rounds of tax cuts, cheaper medicines, cheaper early childhood education, fee‑free TAFE, all of these things are about recognising that when people are under pressure, there is a role for governments to step in and help where they can responsibly do that.

    So from budget to budget, and we’ve had 4 already, and the fifth one will be in May next year, from budget to budget, we see if we can do more, if we can afford to do more to help people with the cost of living, and people can expect that next May, just like they could expect that in the first 4 budgets.

    Journalist:

    Treasurer, Andrew Bragg says that ‘If Mr Chalmers is so sure his unrealised gains tax will apply to Mr Albanese’s pension, he should say exactly how much tax will be paid in the first year of his pension’. Can you nominate that figure?

    Chalmers:

    One of the reasons why nobody takes that guy seriously is because when it comes to the Prime Minister, his pension’s not yet known. Now we don’t know his circumstances into the future.

    He should know, he’s on the Committee that scrutinised the legislation that Andrew Bragg lied about and said didn’t exist. He also said that there’s no allowance in the legislation for defined benefit schemes for politicians.

    Those are lies. And you need to be really careful not just to read out whatever he tweets, because he’s been caught out lying in the last day or so. I would encourage you respectfully not to take his word for it, especially this week, after he’s been caught out lying so egregiously.

    There is provision for defined benefit schemes, there are calculations, those calculations are very similar to the ones that the Liberals and Nationals put in when they changed superannuation in the last term of the government, and that will apply to the Prime Minister, it will apply to any politician who’s got the equivalent of more than $3 million in super.

    Journalist:

    What do you make of Allan Fels’ call for an ACCC Inquiry into Bunnings?

    Chalmers:

    I’ve got a lot of time for Allan Fels, I respect him, I speak with him from time to time, he’s a great person with a substantial record of achievement.

    We’re already acting on competition, funding the ACCC much more substantially, I provided another $30 million to empower one of Allan’s successors in that role, Gina Cass‑Gottlieb, doing a wonderful job, we’ve found more resources for her.

    Our primary focus is on the supermarkets, we’ve made that really clear, price gouging and the Food and Grocery Code, but we have the ability, should we want to, to expand some of that focus, and the extra resources that I provided the ACCC will help ensure that where there’s more work to be done, it can be done.

    Journalist:

    Treasurer, there’s –

    Chalmers:

    We might just take 2 more. One more, and then another Andrew Bragg tweet, and then we’re done.

    Journalist:

    There’s an issue unfolding with the disability company, Cocoon SDA Care that operates partly in your electorate. Do you have any concerns about what’s going on with Cocoon and have any of your constituents raised concerns?

    Chalmers:

    Not that I’m aware of, but I’ll look into that, that’s the first I’ve been aware of that particular issue, but I’ll make sure I look into it, and if there’s anything I can say publicly at some future point, I’ll do that.

    Journalist:

    What do you say to the leading independent economists, just not Gerry Harvey or Andrew Bragg, who say that this will hurt investment, wealthy people will take their investments away from, you know, venture capital and start‑ups, and it could ruin the tech industry?

    Chalmers:

    First of all, there’s not a unanimous view amongst economists about that, or about the worthiness of the change that we’re proposing. I think Chris Richardson, for example, wrote something supporting it, and so always, when you’re making a change like this, there’s always a range of views, and obviously I follow closely the comments made by the peak groups and others.

    It really comes back to the question I gave earlier to your colleague; we’re still providing concessional tax treatment for people with big balances in superannuation, it’s just slightly less concessional, but it’s concessional compared to the marginal rate that people would be paying.

    And so I think we need a little bit of perspective here, I know that this is seen in some quarters as contentious, but again, I mean we announced this policy almost 2 and a half years ago, it’s been in the Parliament for a big chunk of that, we’ve been consulting on it, it’s a modest change, it still leaves concessional tax arrangements in there for people who have more than $3 million in super.

    I expect that there’s a campaign run about it, I expect that people have got views about it, but I do think we need a bit of perspective here. It is a modest change, it does impact only a very small amount of people, and it still provides concessional tax treatment.

    Journalist:

    Treasurer, just on –

    Chalmers:

    I might just take one more here because you’ve been light on, and then we’ll go.

    Journalist:

    Thank you, sir. Just about the wage review again.Have you spoken to the Reserve Bank about the wage review and whether or not it’s inflationary?

    Chalmers:

    Thank you. I have had discussions with the Governor of the Reserve Bank and the Treasury has been engaging with the Assistant Governor as well.

    We wanted to make sure that the submission that we’re putting forward, which is about a sustainable real wage increase for millions of Australians on awards, that that’s consistent with our other objectives, including getting on top of this inflation, which has impacted economies around the world over recent years.

    So I consulted the Governor, I think towards the end of March, I gave her a heads‑up today that we were making our submission today, the Treasury’s been engaging with the Reserve Bank and its staff, and that’s because we have made sure that this is consistent with inflation remaining sustainably in the band; that’s our objective.

    One of the things I’m really pleased about and proud of collectively in our economy, is we’ve managed to get real wages up over a sustained period of time at the same time as we’ve got inflation down, kept unemployment low, got the economy growing again, we’ve seen interest rates started to come down earlier this year, we’ve got the debt down in the Budget, so we’re paying less interest on it.

    So this, I think, does reflect the very substantial progress that Australians have made together in our economy. We know that there’s more work to do because people are under pressure, the global environment is still uncertain, but the submission that we take today reflects all of our economic objectives and primarily making sure that when people work hard, they can get ahead.

    I’ll take one more from you, then we’re done.

    Journalist:

    The ACTU want it to be 4.5 per cent, ARA says no more than 2.5 per cent. Is it somewhere in between that you kind of want it?

    Chalmers:

    It’s unusual, and in fact it’s welcome for different groups, including the union movement, to make submissions to the Fair Work Commission’s process. Those submissions close today, there will be hearings next week, a decision next month, it will kick in in July, and it’s a good and welcome part of the process that everyone’s got the ability to make a submission, like the government has today.

    Some organisations nominate numbers, others like the government don’t nominate numbers. The Fair Work Commission in its wisdom will weigh up all of that and come to a decision.

    Journalist:

    Leaning more towards the union, or the business bodies?

    Chalmers:

    Well, that’s not how we make our submission. You know, we’ve made a detailed submission today. You know, I’ve worked really closely with Amanda Rishworth on it, before that with Murray Watt, before that with Tony Burke. We put a lot of effort, a lot of thinking, we apply a lot of consideration to the submission that we make, we don’t put a number on it like other groups do.

    And I also welcome the fact that when we’ve been through this process on a number of occasions already in the life of this government, that the Fair Work Commission has provided, you know, decent pay increases for Australians who are low paid or on awards. That’s a very good thing, and we hope to see that again. More than that, we hope to see a real wage increase.

  • MIL-OSI Russia: More than 20 doctors from Central Asian countries have been trained in tuberculosis treatment in XUAR

    URUMQI, May 16 (Xinhua) — Four advanced training course participants from the National Center for Tuberculosis Problems of the Ministry of Health of the Republic of Kazakhstan recently completed a month-long training at the Sixth People’s Hospital of Xinjiang Uygur Autonomous Region (XUAR, northwest China).

    Thus, to date, 23 doctors from Central Asian countries have already been trained in the treatment of this disease in the above-mentioned medical institution.

    Over the past month, medical trainees from Kazakhstan have held in-depth discussions on topics such as diagnosis and treatment of drug-resistant tuberculosis and basic interventional bronchoscopy operations, and joined standardized training in complex surgeries and multidisciplinary consultations on difficult-to-treat diseases.

    According to Yerbol Kopbayev, China is impressive in terms of standardized management of the treatment process in case of drug-resistant tuberculosis, the model of interdisciplinary cooperation and the use of advanced diagnostic and treatment equipment, especially the norms and standards for the treatment of complex cases, which provide new ideas.

    At the graduation ceremony of the above-mentioned courses, the leaders of the Xinjiang hospital expressed hope that the medical trainees would provide higher quality medical services after mastering new skills and abilities.

    As it became known, the above-mentioned Chinese medical institution will disseminate technologies for minimally invasive operations in the field of thoracic surgery in Central Asia, and will also continue to provide international remote medical consultations.

    According to the survey data, Xinjiang remained one of the regions in China with a high incidence of tuberculosis from 1979 to 2010. Thanks to the efforts of local authorities, the incidence of tuberculosis in Xinjiang has significantly decreased, from 304.9 cases per 100,000 people in 2018 to 87.8 cases per 100,000 people in 2021. Currently, all local residents aged 15 and above are included in the screening program for tuberculosis symptoms and chest X-rays. -0-

  • MIL-Evening Report: With a new minister for early childhood education, what can the federal government do to make centres safer?

    This week, more reports emerged of horrific abuse of children at childcare centres.
    An ABC investigation reported young children had suffered burns and been verbally abused. In another case, a baby was repeatedly slapped by an early childhood educator.

    This follows claims of sexual abuse and neglect earlier this year.

    On Tuesday, we also saw the appointment of Victorian senator Jess Walsh as the new minister for early childhood education. As a former head of the union responsible for the sector, Walsh comes to the role with a thorough understanding of early education issues. She will also sit in cabinet.

    What does Walsh and the re-elected Albanese government need to do to better ensure children’s safety in early childhood education and care? Here are three ideas.

    1. Design safer centres

    One place to start is how centres are set up.

    “Safety by design” is a concept used in other sectors (such as online safety) and has its origins in crime prevention. It means planning spaces and systems to prevent harm before it happens. This could include changing the environment, routines or rules.

    For example, clear lines of sight in a room help educators watch children, and each other, more easily. Secure entrances ensure only authorised people can come into a centre.

    But safety isn’t just about buildings; it also depends on people. Educators need the right training to spot risks and signs of harm, and to act early.

    This means building their capability: not just knowing what to do, but feeling confident to speak up and raise concerns. This confidence needs to be encouraged by managers and leaders in centres – staff should be supported to speak openly.

    2. Conduct a thorough investigation

    Australia has a system to monitor quality standards in early childhood services via the Australian Children’s Education and Care Quality Authority or ACECQA. This body oversees a National Quality Framework.

    On Friday, National Children’s Commissioner Anne Hollonds said Australia needs to “urgently strengthen” regulatory frameworks.

    The recent reports of abuse and noncompliance certainly raise a series
    of questions: how could this happen? How could “trained professionals” think this treatment of children is OK?

    Multiple systemic failures could be the answer – meaning the mechanisms or processes in place to stop situations going from unacceptable to unfathomable, failed.

    A federally convened taskforce could investigate these breaches of child safety to identify risks and failures and prevent further cases of harm. Rather than yet another long inquiry, a taskforce could help get to the root of the problems and recommend solutions that can be implemented quickly.

    3. Boost the status of early childhood education

    Governments can also invest in the ongoing professionalisation of early childhood educators. They can do this by insisting on higher qualifications, pay, professional development and a strong ethical framework.

    In turn, this can enhance the ability of those in the sector to prevent and respond to abusive practices.

    Admittedly, the federal government has taken steps to increase requirements around qualifications and pay. But early childhood professionals continue to endure low status in the community.

    The people who work in early childhood services are not “babysitters” – they are trained educators. Early childhood education is also more than a means to increase productivity, by enabling parents to work. It provides education and care to children at a crucial time in their development.

    None of these perceptions help the sector or the service it provides. The whole community needs to understand the vital role it plays in our society – just like schools or hospitals.

    If Australians appreciate and value early childhood education, they can advocate for (or at least support) improvements and investments.

    A final word to parents

    It is understandable parents might find recent headlines about abuse and neglect in early childhood centres distressing. Keep in mind, 91% of early childhood services have been assessed as at least meeting national quality standards.

    WASHINGTON – U.S. Senators Bill Cassidy, M.D. (R-LA) and Richard Blumenthal (D-CT) introduced the Medicare Beneficiary Co-Pay Fairness Act of 2025 to make Medicare’s co-pay structure more affordable by extending the existing co-pay cap to Ambulatory Surgery Centers (ASCs), ensuring fairer costs for patients, and supporting the continued growth and utilization of efficient, lower-cost surgical settings.
    “If your grandmother depended on Medicare for life-saving treatment, you would not want to hear that Medicare was cutting corners,” said Dr. Cassidy. “This bill makes costs fairer for patients while keeping the quality of care high.”
    Background
    Currently, while patients in both ASCs and Hospital Outpatient Departments (HOPDs) typically face a 20% co-pay, only HOPDs benefit from a co-pay cap, set at $1,676 for 2025. This leaves Medicare patients utilizing the over 6,300 Medicare-certified ASCs nationwide with potentially higher out-of-pocket expenses for approximately 150 procedures, even though ASCs offer high-quality, cost-effective care projected to save Medicare billions.

    The Nasser Hospital in Khan Younis reported that 54 people, including women and children, were killed in strikes on the southern city, according to a press statement.

    According to Gaza-based health authorities, the Gaza European Hospital, the only hospital providing medical follow-up care to cancer patients in the enclave, was out of service due to recent Israeli attacks.

    The Israeli attacks “caused significant damage to infrastructure, such as sewage lines, damage to internal departments, and destruction of roads leading to the hospital,” the authorities said in a press statement.

    Meanwhile, medical sources told Xinhua that 26 others were killed in Israeli airstrikes on Gaza City and other areas in northern Gaza.

    The airstrikes came after Israeli Prime Minister Benjamin Netanyahu warned Tuesday that the Israeli military would enter Gaza “with full force” in the coming days to press forward with efforts to defeat Hamas.

    Israel resumed large-scale military operations in Gaza on March 18, ending a two-month ceasefire. Since then, 2,876 Palestinians have been killed and more than 7,800 injured, according to health officials in Gaza.

    The total Palestinian death toll since the war erupted on Oct. 7, 2023, has reached 53,010, the officials said on Thursday.

    Israel is using a policy of “reducing space and emptying populated areas to pressure citizens,” Mahmoud Basal, spokesperson for the Civil Defense in Gaza, told Xinhua on Thursday.

    He also claimed that thousands of people spent the night in the streets amid threats of strikes on schools and shelters housing the displaced, adding that Israeli forces were obstructing emergency teams from reaching victims and systematically destroying Civil Defense infrastructure.

    Every year, Australia buries millions of tonnes of waste in landfills. But these sites are filling fast, recycling has its own limitations, and most waste export is banned. So councils and state governments are looking for alternatives.

    Several large-scale incinerators have been proposed, to turn municipal solid waste into electricity. One is already up and running in Perth’s outer suburbs.

    The A$1.5 billion Parkes Energy Recovery project planned for New South Wales would be Australia’s biggest. However, community backlash over potential health risks could put the plan in doubt.

    As chemical engineers, we recognise the potential benefits of this technology. Modern facilities operating around the world show these processes can be efficient, safe and environmentally controlled. However, minimal risk does not mean zero risk. Understanding both the benefits and challenges is crucial to address community concerns.

    What is waste-to-energy?

    Waste-to-energy, also known as energy-from-waste, can transform waste otherwise destined for landfill into electricity, heat or fuel.

    This does not replace recycling. Instead, it offers a solution for materials that are difficult or impossible to recycle. Care must be taken, however, to ensure waste-to-energy technologies complement rather than supplant recycling efforts.

    How does it work?

    There are three main types of waste-to-energy technologies:

    1. Thermal: use heat to generate steam, which spins turbines to create electricity. The heat can come from burning waste, producing carbon dioxide, water and ash. Alternatively, solid waste can be turned into gas (hydrogen and carbon monoxide). This process is known as gasification.

    2. Biological: use microorganisms to break down organic matter in the waste stream, producing biogas, mainly methane. This is then used for power or heat generation.

    3. Chemical: use processes such as pyrolysis or hydrothermal liquefaction to convert hard-to-recycle materials into fuels or chemicals. These can feed into industrial and manufacturing processes.

    What’s holding Australia back?

    When most Australians hear about making energy from waste, they think of
    old-fashioned incinerators. Those outdated facilities released smoke and toxins into the air.

    But modern incinerators use advanced air pollution control systems that capture harmful emissions.

    Some use static electricity to remove dust or smoke particles from the gas stream. Other pollution control systems include acid gas scrubbers, catalytic converters and fabric filters.

    This can cut emissions of fine particles by up to 99%.

    The volume of waste sent to landfill is also reduced by up to 90%. What remains includes incinerator bottom ash and fly ash. Often these can be reused in making concrete, pavement and other construction materials. But regulatory issues will need to be overcome before this can happen in Australia.

    Introducing the Parkes project

    The Parkes Energy Recovery project, announced in March, promises to process around 600,000 tonnes of waste a year. This should generate at least 60 megawatts of electricity – enough to power 80,000 homes.

    To receive development approval, the project must comply with stringent environmental and health standards. This includes preparing an Environmental Impact Statement and Human Health Risk Assessment. The NSW Environment Protection Authority may then issue an Environment Protection Licence. Such a licence requires ongoing monitoring and frequent audits.

    Extensive community consultation is underway.

    Other projects around Australia

    There are two waste-to-energy plants in Western Australia, one at Kwinana and another under construction at East Rockingham. A third plant has been given the go-ahead in Victoria, at Maryvale.

    Kwinana received its first delivery of waste in July 2024.

    Licences to build other major waste-to-energy facilities have been issued in Victoria. Various proposals are also being considered in New South Wales, Queensland and South Australia.

    Taking tips from overseas

    A shortage of landfill sites in cities across Europe and Asia originally promoted investment in waste-to-energy technology. These power plants are now commonplace in Germany, the Netherlands and Japan, substantially reducing reliance on landfill.

    The Amager Bakke plant in Copenhagen shows how such facilities can also enrich a community. This award-winning building doubles as a public recreation space, complete with a rooftop ski slope.

    In China, the proposed Shenzhen East Waste-to-Energy Plant could process 5,000 tonnes of waste a day. That works out to 1.8 million tonnes of waste a year, if run continuously.

    Waste-to-energy and the circular economy

    Waste-to-energy technology is useful in the transition to a circular economy. This is an economy where resources are continually cycled through the system and never wasted.

    Reusing, recycling and reducing waste must remain top priorities. Waste-to-energy technology should then be used as a last resort, extracting value from hard- or impossible-to-recycle materials.

    It’s certainly better than sending waste to landfill. When buried underground, waste can leach toxins into soil, ground and surface water. The potent greenhouse gas methane is also released when food rots in landfill.

    Over-reliance on waste-to-energy could supplant more sustainable circular recycling efforts. But incineration plants are being scaled back in Europe, as the focus shifts to reuse.

    The case for waste-to-energy

    Despite its potential, waste-to-energy technology remains controversial in Australia. Some local communities remain concerned about emissions and potential long-term health risks. Environmental groups also question the potential effects on recycling rates.

    Nevertheless, growing awareness of the limitations of recycling, increasing landfill levies, bans on waste exports, and ambitious federal and state circular economy strategies are making waste-to-energy a more pragmatic option. Stringent regulation and community consultation will be necessary to get these projects off the ground.

    Responsible use of modern waste-to-energy technology can generate electricity and heat for homes with minimal emissions, and can extend benefits that serve local communities. It can also complement Australia’s renewable energy targets while taking a better approach to managing waste.

    Background:
    The Empower Charter School Educators to Lead Act would:
    Authorize state entities receiving Federal Charter Schools Programs (CSP) grants to make pre-planning awards in amounts of no more than $100,000 to prospective applicants, or public or nonprofit entities that will support prospective applicants, provided they:
    Are led by educators with 4.5 years of school-based experience;
    Have successfully completed the development of an initial plan for the opening of a public charter school;
    And have not yet submitted a proposal for approval of a charter to an authorized public chartering agency.

    Permit state entities to use up to 5% of their grant funds for those pre-planning awards;
    Raise the cap on the percentage of state entities’ grant funds that may be used for technical assistance and quality improvement activities from 7% to 10%;
    Raise the cap on the percentage that may be used for state administration from 3% to 5%;
    And clarify that states’ technical assistance activities may include assistance in locating and accessing a facility.
    This legislation is endorsed by the National Alliance for Public Charter Schools, National Associations of Charter School Authorizers, KIPP Public Schools, and Center for Learner Equity.

    Manama – The International Organization for Migration (IOM) Bahrain was honored to join the celebration of Pravasi Legal Cell’s (PLC) third anniversary held on Tuesday Evening at KIMSHEALTH Hospital in Umm Al Hassam. IOM was graciously presented with an award in recognition of its partnership in supporting the rights of migrant workers in the Kingdom of Bahrain.

    Over the past year, IOM Bahrain has partnered closely with PLC along with a wide range of community leaders and organizations. Through initiatives like the Working Together awareness campaign, IOM has collaborated with the LMRA and key partners, including PLC, to support vulnerable migrants and raise awareness about the rights and responsibilities of both workers and employers in the Kingdom.

    Additionally, IOM and PLC have partnered on grassroots capacity building, including training community leaders on suicide prevention and raising awareness around the rights and responsibilities for employers of domestic workers. PLC has remained a consistently motivated and engaged presence in these workshops and trainings, embarking on a long-term journey to tackle priority areas in the community where collective efforts can make a significant impact in the future.

    A Police investigation is ongoing after a man was seriously wounded in Papakura before lunchtime.

    The incident occurred at around 11.42am, when the victim suffered an injury outside a business on Settlement Road.

    Detective Senior Sergeant Michele Gillespie, of Counties Manukau CIB, says the victim sustained serious but not life-threatening injuries and was transported to Middlemore Hospital.

    “The offender has fled on foot across Settlement Road and was last seen near an address,” she says.

    Police were called to the scene shortly afterward with nearby schools placed into a precautionary lockdown with the offender outstanding.

    “Cordons were put in place while our staff carried out enquiries in the area to locate this male,” Detective Senior Sergeant Gillespie says.

    “Those cordons have now lifted, and the schools were taken out of lockdown earlier this afternoon.

    “Our enquiries are ongoing to locate this male.”

    Police acknowledge the community’s cooperation this afternoon.

    “Police are continuing to investigate the circumstances of what has occurred today, including locating the offender.”

    In the 2000s, biotech company Theranos promised to revolutionise blood testing. Founder Elizabeth Holmes claimed Theranos technology could perform hundreds of tests using just a finger-prick drop of blood. If true, their diagnostics would be faster, cheaper and more accessible.

    Theranos raised hundreds of millions of dollars from investors and was valued at more than US$9 billion in 2015.

    However, the technology never worked, leading to one of the biggest scandals in biotech history. Theranos was secretly using traditional machines to run many tests, then claiming the results came from its own (non-functional) device. Holmes was eventually convicted of fraud and sentenced to 11 years in prison.

    Today, a new startup, Haemanthus, claims to have developed a similar technology. Co-founded by Billy Evans (Holmes’ partner), this new company says it can detect and diagnose illnesses using tiny amounts of blood, urine, or even saliva.

    While technology has advanced since Theranos’ time, it’s important to consider these claims carefully.

    Clinicians and lab techs can currently detect many conditions with blood, and some with urine or saliva. These are an important tools in modern medicine. However, the volumes required are usually much greater than a few drops or a dab.

    What can blood detect?

    Blood circulates through all organs, transporting cells, nutrients, hormones and waste products. Blood tests collect several millilitres of blood from a vein and send this to a laboratory for analysis.

    Blood tests can check if a person has signs of infection or disease, to monitor organ function, or to show how a person is responding to medical treatment. Blood tests are widely used to monitor heart disease, diabetes, kidney disease, or deficiencies in iron or vitamins.

    A significant proportion of medical decisions are based on laboratory analysis of blood tests. Making them more affordable and accessible would have great benefits.

    What about urine?

    Urine is produced by the kidneys and contains waste filtered from the blood. The colour and composition of urine gives you a snapshot of any problems the body might be trying to fix.

    Urine analysis can detect urinary tract infections, kidney disorders, diabetes and liver diseases by measuring sugars, proteins and cells.

    As urine tests are non-invasive and easy to administer, they can be used to quickly screen for some conditions.

    However, factors such as how much you’ve had to drink and what you’ve eaten can influence urine composition, potentially affecting test results.

    Saliva can also be used for diagnoses

    Saliva is the clear, watery liquid produced by salivary glands in the mouth. It’s mostly water (around 99%), but also contains various substances such as hormones, antibodies, enzymes, DNA, RNA and metabolites.

    Saliva testing is already used in clinical settings to detect HIV antibodies, monitor levels of cortisol (a marker of stress) and to diagnose viral infections such as COVID.

    The potential of saliva as another non-invasive diagnostic tool is growing, especially as researchers identify more markers of disease that it can contain.

    However, saliva production varies between individuals. The composition of saliva can be impacted by what you eat and drink, the time of day, or even stress. These variations can limit how consistent and reliable saliva can be for making a diagnosis.

    But how much of it do you need?

    While diagnosing diseases using bodily fluids isn’t new, Haemanthus and other startups differentiate themselves by aiming (and claiming) to need only small amounts for multiple tests: a drop of blood, a swab of saliva, or a few milliliters of urine. This would mean faster, cheaper, more convenient tests that cause less discomfort.

    But there are physical limitations of small samples. Many diagnostic markers (called biomarkers) are only present in very low amounts in our body fluids.

    When the sample amount decreases, so do the amounts of the biomarkers, making it harder to detect them reliably. This is particularly true for biomarkers such as hormones, cancer markers, or early-stage disease indicators.

    What can you detect with a few drops?

    Of course, some conditions can be diagnosed using small samples, but generally only one condition is tested for with each small sample, unlike the claims of companies like Theranos.

    Finger-prick blood samples, for example, can monitor blood glucose levels of people with diabetes.

    Small urine samples can detect urinary tract infections, but not all types of infections at once.

    The specific biomarkers for these conditions can be reliably detectable in small amounts of fluid.

    To diagnose more complex conditions, or even unknown conditions, multiple tests may be required, each needing different sample preparations. This requires both volume and precision – two things tests with small sample volumes struggle to deliver.

    What happens next?

    While the idea of diagnosing illnesses with small fluid samples is promising, especially for remote or resource-limited settings, the science suggests we should be cautious.

    Most human diseases are complicated, and we usually need comprehensive testing approaches to diagnose them. Relying only on small fluid samples could lead to misdiagnosis, delayed treatments, or unnecessary interventions.

    Innovations in biosensor technology, machine learning algorithms and biomarker discovery continue to advance the field. And one day, fast and reliable small volume testing may be possible.

    However, a lot more peer-reviewed research and regulatory approvals will be essential to ensure patient safety and diagnostic accuracy.

    On election night, a triumphant Anthony Albanese took to the stage brandishing a Medicare card as a symbol of the nation’s commitment to public healthcare.

    As the re-elected government gets to work on its promised national food security strategy “Feeding Australia”, it has a unique opportunity to build a strategic agenda as bold and transformative as Medicare.

    That agenda is investment in food as a public good – a recognition that a healthy food system is as important to the nation’s health and wellbeing as access to hospitals, bulk-billing doctors and subsidised medicines.

    Feeding Australia

    The new Labor government, with its large majority, has a once-in-a-generation chance to deliver meaningful change in our food system.

    It went into the election promising a new food security strategy, which Agriculture Minister Julie Collins says will improve supply chain resilience and and minimise price volatility at the checkout:

    Australia has an impressive record in agriculture, feeding millions of people both here and abroad, but we can’t afford to be complacent. The Albanese Labor government will protect and strengthen Australia’s food security for the benefit of our farmers and all Australians, as well as the trading partners that rely on our produce. When our food and supply chains are secure, it reduces financial strain on households, helping all Australians.

    Labor has tried this before. In 2013, the Gillard government’s short-lived National Food Plan was critcised for prioritising corporate interests over public health and sustainability.

    Repeating past mistakes will again risk putting corporate hunger first. The Feeding Australia strategy must prioritise the health of people, planet, and care for Country.

    Food for thought

    The food security strategy must address multiple, converging crises:

    • growing food poverty
    • worsening diet-related health
    • biosecurity threats
    • accelerating climate change
    • declining farmer viability
    • supermarket duopoly.

    Australia produces enough food to feed more than twice its population. Yet it struggles to feed its own people well.

    Foodbank Australia estimates one third of Australians now experience some form of food insecurity. A combination of market failures and policy inaction leaves us vulnerable to supply chain disruption and even greater food inequity.

    Biosecurity is also a challenge. The recent outbreak of bird flu means eggs – a basic pantry item – now cost 16.1% more than 2020.

    But it’s not only consumers who are suffering. One-third of vegetable growers are considering leaving agriculture in the next year, due to high costs and what growers’ group AUSVEG has called the “relentless squeeze” on margins.

    A business-as-usual approach will only reinforce the current state of Australia’s supermarket sector, which is among the most concentrated and profitable in the world. Accusations of price gouging and misleading pricing raise concerns for consumers, particularly during a cost-of-living crisis.

    As extreme climate events and biosecurity threats increase in frequency and intensity, the duopoly’s centralised supply chains have occasionally failed. After this year’s floods in Far North Queensland, supermarket shelves were empty once again.

    Yet, independent grocers with shorter supply chains remained stocked – as they did during the Queensland floods in 2011.

    The food strategy must do more than offer a band-aid solution to fix an ailing food system.

    Community networks

    Local food networks have an important role to play in this process.

    They are collectives of people and organisations that are committed to creating food and farming systems that put health, equity, and sustainability first. They gather collective wisdom, mobilise public procurement to support local producers, and secure more democratic, health-oriented, and sustainable food system policies.

    Food networks are flourishing in North America, which has more than 300 active councils as of 2023. The Australian sector is not as mature, but is growing.

    Groups including the South Australian Urban Food Network, Tasmanian Food Security Council, Southern Harvest (NSW/ACT), and Farm 2 Fork Collective (Queensland), demonstrate growing capacity for citizen involvement in food policy and decision making. These networks encourage local initiatives such as community gardens, food hubs, and localised institutional procurement.

    New research points to how community-led food cooperatives can also help improve food security and healthier diets.

    These, and other examples, show the power of community in strengthening food system resilience and security. But they can’t do it alone. Communities need government support and investment.

    Future food

    The question of who feeds Australia – and how we are fed – matters to us all.

    The National Food Security Strategy is an opportunity to forge a more healthy food future. It can lay the foundations for a food and farming system that feeds us well for generations to come.

    Achieving this bold agenda will take an inclusive, participatory process that foregrounds First Nations’ voices and the lived experience of those at the sharp end of the cost-of-living crisis.

    During the event, thematic specializations of the campus were defined and focused in accordance with the strategic priorities for the development of the country, industry and region. Participants of the strategic session analyzed in detail the research areas, flagship products and their potential for development based on the modern infrastructure of the NSU campus. As a result, comprehensive product programs were developed in such areas as applied mathematics (including artificial intelligence and big data processing), applied engineering, biotechnology and biomedicine, new functional materials, as well as space instrumentation.

    — Our joint task is to fill the new buildings with advanced scientific developments, high-quality education and unique technologies in demand by all campus users in the shortest possible time: both the regional leadership and industrial partners, as well as students, teachers and city residents. The strategic session in Novosibirsk showed excellent results, demonstrating coordinated, organized and constructive work. The teams created promising products focused on the interests and needs of modern youth.     The strategic session was attended by about 100 NSU employees representing key areas of the university, including space instrumentation, biotechnology and biomedical research, as well as advanced areas of mathematics. In addition, representatives of the Novosibirsk Region Government and industrial partners of the campus were invited to participate.

    Let us recall that Novosibirsk Oblast is among the five regions that will be the first to develop and implement product programs. In recent years, the university’s strategy has been transformed towards building closer interaction with economic sectors and industrial partners. The development of a campus product program is an important step in implementing NSU’s development strategy and will allow the university to strengthen the campus’s position as a leading scientific and educational center.

    The construction of the NSU campus includes two stages: the first — the educational building and leisure center of the NSU SUNC, as well as the NSU dormitory complex for 690 people — was put into operation in May 2024 and opened its doors to students in September 2024. The construction of the second stage has crossed the “equator” — the overall readiness of the facilities is 57%. The building of the flow auditoriums was put into operation in December 2024, its furnishing with furniture and equipment will be completed in the second quarter of 2025, the educational process in the new building will begin in September of this year. The buildings of the educational and scientific center of the NSU Institute of Medicine and Medical Technologies and the research center are also being erected. Their construction is planned to be completed in 2026.

    On the instructions of President Vladimir Putin, a network of modern campuses is being created in Russia. By 2030, a constellation of 25 campuses should appear in the country. Work in this area is being carried out by the Government of the Russian Federation and the Ministry of Education and Science of Russia. Currently, 24 such campuses are being designed and built with the support of the national project “Youth and Children”. One of them has already been completely built in Moscow on the basis of the Bauman Moscow State Technical University. By 2036, the number of campuses will increase to 40. The project is being financed by federal and regional budgets, as well as by extra-budgetary sources.

    People from the LGBTIQA+ community who attend a breast screening appointment in the ACT will receive a rainbow scarf to foster inclusivity and encourage health screenings.

    BreastScreen ACT has today launched the Inclusive Rainbow Threads Project ahead of the International Day Against Homophobia, Biphobia, Intersexism, and Transphobia.

    In collaboration with local artist LaToya Kennedy of Kalari Art, BreastScreen ACT has created vibrant rainbow scarves to encourage people from the LGBTIQA+ community to invest in their health and book in for a breast screen.

    Minister for Health Rachel Stephen-Smith emphasised the importance of creating a safe and welcoming environment for all individuals accessing breast screening services in the ACT.

    “The Inclusive Rainbow Threads Project is a positive step towards ensuring that breast and chest screenings are accessible and inclusive for everyone, regardless of gender identity or sexual orientation,” Minister Stephen-Smith said.

    “The rainbow scarves will be given to individuals who identify as part of the LGBTIQA+ community and attend their breast screening appointment.

    “By fostering an environment of support and respect, we aim to encourage regular screenings and promote the health and wellbeing of the LGBTIQA+ community in the ACT.”

    The project is supported by BreastScreen ACT’s community of donors and supporters, Luton Properties, the ACT Government LGBTQIA+ and Social Inclusion Strategy Team, and the Canberra Hospital Foundation.

    Through Canberra Hospital Foundation’s Pay It Forward initiative, members of the community can contribute by purchasing a scarf where a portion of the funds will pay it forward for someone in the LGBTQIA+ community who attends a screening.

    This project builds on the installation of three new mammography machines at ACT community health centres in 2022-23.

    Free mammograms are available to eligible Canberrans over the age of 40 years. People aged 50 to 74 years are actively encouraged to have mammograms every two years. It only takes about 20 minutes and no referral is needed.

  • MIL-Evening Report: Some young trans people take sex hormones so their bodies better align with their gender. What are the benefits and risks?

    Triggered by hormonal changes in the brain and body, puberty marks a physical transformation. Oestrogen and testosterone – often called “sex hormones” – drive many familiar changes, such as breast development and periods or a deeper voice and facial hair.

    For most young people, the pubertal changes they experience align with their gender. However, for trans and gender diverse adolescents, these changes can be distressing and may lead to a sense of disconnection from their true sense of self.

    (We’ll use the term “trans” in this article to refer to transgender and gender diverse people.)

    Why do young trans people use sex hormones?

    To support trans adolescents, oestrogen and testosterone may be used to induce physical changes that better align with their gender.

    Oestrogen stimulates breast development, alters body fat distribution, results in softer skin, and reduces facial and body hair, creating a more feminine appearance.

    Testosterone deepens the voice, increases facial and body hair, promotes muscle growth and stops periods, creating a more masculine appearance.

    Hormones are just one means by which people can affirm their gender and only some trans adolescents seek sex hormone treatment. Other means include puberty suppression, changing your name, pronouns, hair, clothing and legal documents.

    When do trans people start sex hormones?

    Many trans people start taking sex hormones as adults.

    For those who start as adolescents, hormones are introduced when a young person is considered to have the intellectual and emotional maturity to make this decision.

    The starting age also varies depending on the person’s preferences, family support and barriers to accessing care. These barriers include long wait times, regional disparities, costs, legal or policy restrictions, and challenges navigating the health system, all of which make it harder to get timely, reliable care.

    Before starting hormone therapy, trans adolescents undergo comprehensive assessment and counselling with a team of mental health professionals and medical doctors who specialise in transgender health. This helps ensure young people understand the potential benefits, limitations, risks, and long-term implications of treatment.

    This process involves their family and is designed to provide time, support and space for shared, informed decision-making.

    Treatment with sex hormones usually begins with low doses, after which adjustments are made over time under regular clinical monitoring.

    Subsequent physical changes occur gradually over several years, as is true for puberty, and some of these are irreversible. Breast growth or a deepened voice, for example, will persist if treatment is stopped.

    What are the benefits?

    The largest study to date followed 315 trans and gender diverse adolescents for two years after starting oestrogen or testosterone. It found a significant increase in how comfortable participants felt about their appearance. This was accompanied by significant improvements in life satisfaction and reductions in depression and anxiety.

    More recently, Australian youth mental health research centre Orygen conducted a review of the overall evidence. It reported sex hormone treatment for young trans people is associated with body image satisfaction and reduced psychological symptoms, including depression, anxiety and suicide attempts, thoughts and/or planning.

    An independent review of the evidence commissioned by NSW Health reported similar conclusions and found sex hormone therapy was associated with reduced gender dysphoria, which is the distress experienced when a person’s gender identity differs from their sex reported at birth.

    Together, these published outcomes are consistent with decades of clinical and lived experience that young trans people report feeling more at ease in their bodies, more confident in social settings, and more optimistic about their future after starting hormones.

    What about unwanted effects?

    Like all medical treatments, sex hormone therapy involves the potential for unwanted effects.

    Oestrogen, for example, can increase the risk of blood clots and breast cancer in the long-term, but the overall risk appears low.

    Testosterone can increase acne as well as the number of red blood cells the body produces. Too many red cells can make a person’s blood too thick and increase the likelihood of a stroke or heart attack. Monitoring red cell counts and adjusting the dose of testosterone helps reduce these risks.

    Oestrogen and testosterone can also affect the reproductive system. Oestrogen can stop sperm production and testosterone can stop the ovaries from releasing eggs, but neither treatment should be considered an effective form of contraception.

    Given the possibility that trans adolescents might wish to stay on hormone treatment long-term, they are usually offered fertility counselling before starting treatment. The option to freeze sperm or eggs exists, although access and affordability can be a challenge.

    One unwanted effect that has received a lot of attention relates to regret. There is fear adolescents who start hormone treatment will regret this decision later on.

    At this stage, the risk of regret among adolescents starting hormone therapy appears low. Harvard researchers recently followed a cohort of 1,050 adolescents who had received hormone therapy, and only one was noted to have expressed regret.

    However, the risk of regret is inherent to any medical treatment. Allowing adolescents the dignity of risk to make their own decisions respects their autonomy.

    Sex hormones have been used in trans adolescents since at least the 1980s, and so far the evidence suggests these treatments are safe and work well for those who receive them. Nonetheless, long-term research into their safety (and effectiveness) is ongoing and essential.

    What about consent?

    Past decisions of the Family Court of Australia established a requirement for both parents to provide consent for their trans adolescent to access hormonal treatments, treating this care as legally exceptional.

    However, a 2022 decision of the Queensland Supreme Court ruled an adolescent, who is under the age of 18 and has sufficient understanding and intelligence to consent for themselves, could consent to their own hormone treatment.

    Despite this, some gender clinics still require consent from both parents. If there’s a dispute between parents, the matter should be taken to court, to reach a resolution based on the adolescent’s best interests.

    Testosterone and oestrogen therapy are not new or experimental. They are grounded in decades of clinical practice and growing evidence. What is new is the public attention. We need to ensure policies and conversations are based on facts, not fear.

  • MIL-OSI USA: Reconciliation Recommendations of the House Committee on Education and Workforce

    Legislation Summary

    H. Con. Res. 14, the Concurrent Resolution on the Budget for Fiscal Year 2025, instructed the House Committee on Education and Workforce to recommend legislative changes that would decrease deficits by not less than a specified amount over the 2025-2034 period. As part of the reconciliation process, the House Committee on Education and Workforce approved legislation on April 29, 2025, with provisions that would decrease deficits over that period.

    The reconciliation recommendations of the House Committee on Education and Workforce would amend the federal student aid programs authorized by the Higher Education Act of 1965. Specifically, the legislation would modify the federal student loan program by changing repayment terms, loan limits, and requirements for institutional eligibility and alter eligibility for the Federal Pell Grant Program. The legislation also would limit the administrative authority of the Department of Education, repeal certain regulations, and create a new institutional grant program funded through payments from postsecondary institutions.

    Estimated Federal Cost

    The reconciliation recommendations of the House Committee on Education and Workforce would decrease deficits by $349.1 billion over the 2025-2034 period, CBO estimates. The estimated budgetary effect of the legislation is shown in Table 1. The costs of the legislation fall within budget functions 500 (education, training, employment, and social services) and 700 (veterans benefits and services).

    Budgetary Treatment of Federal Student Loans and Pell Grants

    CBO estimates that enacting the legislation would affect spending both for the federal student loan program and for the Federal Pell Grant Program. Those programs are treated differently in the federal budget than most other federal programs.

    Federal Direct Student Loan Program. As required by the Federal Credit Reform Act of 1990 (FCRA), the costs of the federal student loan program are estimated on a net-present-value basis. A present value is a single number that expresses a flow of current and future payments or receipts in terms of an equivalent lump sum paid or received at a specific time. The value depends on the rates of interest, known as the discount rates, used to translate future cash flows into current dollars. FCRA specifies those discount rates as the rates on Treasury securities with similar terms to maturity. As required by FCRA, changes to the estimated costs of outstanding student loans are shown in the year of the enactment of legislation that modifies their terms. The administrative costs of the student loan program are estimated on a cash basis.

    Federal Pell Grant Program. Pell grants provide need-based aid to undergraduate students; they are funded both through discretionary appropriations and through direct spending. For the 2024‑2025 academic year, which began on July 1, 2024, the maximum award funded by discretionary appropriations that a student can receive is $6,335. The discretionary maximum award amount, and the amount of discretionary funding, are set in the annual appropriation act. CBO’s estimate of the program’s cost is based on an assumption that the maximum award will stay the same through 2034.

    The program also has direct spending authority to support a “mandatory add-on,” which increases the award amount by $1,060 above the discretionary maximum. As a result, for the 2024-2025 academic year, the total maximum award is $7,395.

    The bulk of the Pell Grant Program is subject to the appropriation of federal funds. Although CBO anticipates that implementing the legislation would reduce spending subject to appropriation for the discretionary portion of the program, we have not reviewed the legislation for effects on spending subject to appropriation. Only changes to the cost of the mandatory add-on are included in the estimate.

    Direct Spending

    CBO estimates that enacting the legislation would decrease direct spending outlays, on net, by $349.1 billion over the 2025-2034 period (see Table 2).

    Subtitle A. Student Eligibility

    Subtitle A would amend eligibility for federal student aid based on immigration status and adjust the formula for determining the amount of federal aid for which students and their parents would be eligible.

    CBO estimates that enacting subtitle A would decrease direct spending outlays by $518 million over the 2025-2034 period.

    Changes to Aid Eligibility for Certain Immigrants. The legislation would prevent certain aliens (non-U.S. nationals) from receiving federal student aid, including asylees, refugees, Haitian entrants, certain Cuban parolees, T nonimmigrants (trafficking victims), and certain aliens who are victims of domestic violence.

    Overall, CBO expects that enacting this provision would reduce the number of students receiving federal student aid by less than 1,000 each year. Most of the reduction in eligibility would come from Haitian entrants (roughly 70 percent). On that basis, CBO estimates that enacting this provision would reduce direct spending outlays by $15 million over the 2025‑2034 period: $7 million from reductions in the cost of federal student loans and $8 million from reductions in the mandatory add-on for Pell grants.

    Amending Eligibility for Federal Aid. The legislation would cap the total amount of federal aid a student can receive annually at the median cost of college, defined as the median cost of attendance for students enrolled in similar programs. Because loan limits under current law for subsidized and unsubsidized loans are lower, on average, than the median cost of college for most programs, CBO expects that enacting this provision would mostly affect eligibility for parent PLUS and grad PLUS loans. Under current law, students and parents in those programs may borrow up to their institution’s cost of attendance. Using data from the National Postsecondary Student Aid Study (NPSAS) and the National Student Loan Data System (NSLDS), CBO expects enacting this section would reduce annual grad PLUS borrowing by 8 percent and parent PLUS borrowing by 13 percent, primarily for borrowers with the highest cost of attendance.

    In CBO’s estimation, borrowers in the parent PLUS program pay more in principal and interest than they borrow (on a net-present-value basis). On that basis, CBO expects that reducing parent PLUS volume would increase costs to the government. Conversely, CBO estimates that borrowers of other student loans (including grad PLUS loans), on average, repay the government less than they borrowed (on a net-present-value basis). Thus, reducing lending in those programs decreases costs to the government. CBO expects that enacting the provision would reduce net outlays for student loans by $520 million over the 2025-2034 period.

    The legislation also would exclude farm and small business assets from the Student Aid Index (SAI) calculation for Pell grants, generally increasing award levels for students with those assets. Data from a sample of Pell grant recipients indicates that only a small number of recipients or their families own farms or small businesses. CBO estimates that enacting the provision would increase direct spending outlays for Pell grants by $17 million over the 2025-2034 period.

    Subtitle B. Loan Limits

    Beginning July 1, 2026, subtitle B would convert subsidized loans into unsubsidized loans and eliminate the grad PLUS loan program, restrict lending under the parent PLUS program, and amend all annual and aggregate loan limits.

    CBO estimates that enacting the provisions in subtitle B would reduce direct spending outlays by $51.2 billion over the 2025-2034 period. Those savings are estimated on a net-present-value basis and shown in the years in which the loans are originated.

    Eliminate Subsidized Loans and Increase Unsubsidized Loans.The legislation would eliminate subsidized loans and expand borrowing in the unsubsidized loan program for new borrowers starting in academic year 2026-2027, and for all borrowers starting in the 2029‑2030 academic year.

    Under current law, subsidized loans do not accrue interest while the borrower is enrolled in school or in the six months before entering repayment, during the first three years of enrollment in certain income-driven repayment (IDR) plans, and during certain deferment periods. CBO projects that under current law students will borrow roughly $20 billion annually in subsidized loans over the 2026-2034 period. Converting those loans to unsubsidized loans would reduce the cost to the federal government by increasing the interest that borrowers pay on their loans. CBO expects that most students who currently borrow in the subsidized loan program would continue to borrow the same amount in the unsubsidized program. Enacting this provision would reduce outlays by $20.2 billion over the 2025-2034 period, CBO estimates.

    Eliminate Grad PLUS Loans and Amend Limits for Unsubsidized Graduate Loans. The legislation would eliminate grad PLUS loans for new graduate borrowers starting in academic year 2026-2027, and for all borrowers starting in the 2029-2030 academic year.

    The legislation also would amend annual and aggregate loan limits for graduate students in the unsubsidized graduate loan program. Specifically, the legislation would allow graduate students to take out unsubsidized loans up to the median annual cost of their program, with an aggregate maximum of $100,000, or $150,000 if the borrower is enrolled in a graduate professional program. Under current law, graduate students may borrow up to $20,500 each year in unsubsidized loans (with a total aggregate cap for most borrowers of $138,500), and they can borrow up to the cost of attendance in grad PLUS loans, which do not have an aggregate cap.

    Under current law, CBO estimates that borrowers will take out roughly $19 billion in grad PLUS loans annually over the 2026-2034 period. Based on an analysis of current borrowing patterns in NPSAS and NSLDS, CBO expects that students who would have borrowed in the grad PLUS program under current law would instead borrow in the graduate unsubsidized program, up to the new limits.

    CBO expects that enacting both provisions would increase unsubsidized graduate borrowing by 25 percent. On that basis, CBO estimates that eliminating grad PLUS loans and amending unsubsidized loan limits for graduate borrowers would reduce outlays by $34.7 billion over the 2025‑2034 period.

    Restrict Parent PLUS Borrowing and Amend Undergraduate Loan Limits. Beginning on July 1, 2026, the legislation would cap parent PLUS loans at the student’s cost of attendance, by program, minus the maximum in unsubsidized loans the student may borrow in a given year. Students would be required to take out that maximum amount before their parent could borrow under the parent PLUS program. The legislation would set an aggregate cap of $50,000 for parent PLUS loans. There is no aggregate cap on parent PLUS borrowing under current law.

    Additionally, beginning on July 1, 2026, the legislation would allow undergraduate students regardless of dependency status, to take out unsubsidized loans up to the median cost of college for their program of study in a given year, minus any amount awarded in a Pell grant for that year. The aggregate borrowing limit for all undergraduate borrowers would be $50,000.

    Under current law, dependent and independent undergraduate students are subject to different annual and aggregate loan limits based on their class level in school and dependency type. On average, the median cost of college exceeds the current annual loan limits for dependent and independent students. Those current aggregate limits are $31,000 for dependent students and $57,500 for independent students.

    Under current law, CBO estimates that parent PLUS borrowers will take out an average of roughly $13 billion in loans annually over the 2026-2034 period. Under the loan limits specified in the legislation, CBO estimates that parent PLUS borrowing would total roughly $4 billion annually, on average, over the same period.

    The legislation also would permit institutions to cap annual loan amounts according to a student’s program of study, as long as that limit is applied consistently to all students enrolled in a given program. Using information from financial aid associations and other sources, along with data from NPSAS, CBO expects that, under the new loan limits, this provision would limit some of the otherwise expected increase in lending.

    Finally, the legislation would treat pilot-training programs as professional programs, allowing those undergraduate students to borrow up to $150,000. (Currently those students can borrow up to the amount set for their undergraduate aggregate cap, based on dependency).

    CBO estimates that the increases in limits on undergraduate unsubsidized loans, in combination with the restrictions on parent PLUS loans and other provisions, would increase undergraduate borrowing in the unsubsidized program by roughly 15 percent.

    In CBO’s estimation, borrowers in the parent PLUS program pay more in principal and interest than they borrow (on a net-present-value basis). Thus, CBO expects that reducing parent PLUS volume would increase costs to the government. Conversely, CBO estimates that borrowers of undergraduate loans, on average, repay the government less than they borrowed (on a net-present-value basis). Thus, increasing lending of undergraduate loans increases costs to the government. CBO estimates that enacting those provisions together would increase outlays for student loans by $19.1 billion over the 2025-2034 period.

    Set Annual Loan Limits by Enrollment Intensity.The legislation would reduce annual loan limits for undergraduate and graduate loans for students who are not enrolled full time in proportion to their hours of enrollment. Under current law, students enrolled at least half time (for example, six credit hours per semester) are eligible for the full annual loan amounts. Using data from NPSAS and NSLDS, CBO expects that this provision would reduce the volume of loans made to students by about 5 percent and reduce outlays by $15.4 billion over the 2025‑2034 period, relative to current law.

    Subtitle C. Loan Repayment

    The legislation would amend repayment terms for current and new student loan borrowers by limiting income-driven repayment options and extending terms for standard plans based on the amount of debt a borrower holds.

    CBO estimates that those changes would reduce direct spending outlays for student loans by $294.6 billion over the 2025-2034 period.

    For this analysis, CBO used survey data from NPSAS and administrative data from NSLDS. The agency supplemented that information with other data as inputs to project borrowers’ lifetime earnings and repayment of loans. CBO also consulted with a range of experts on postsecondary student aid and reviewed literature on postsecondary enrollment and borrowing.

    Loan Repayment for New Loans.Under the legislation, the Department of Education would offer borrowers two repayment plans for loans originated after June 30, 2026: a standard repayment plan and a new IDR plan. The legislation would eliminate all other plans, including the Saving on a Valuable Education (SAVE) Plan, the IDR plan created administratively in 2023.

    Loans entering repayment would automatically be enrolled in a standard repayment plan, with the length of the repayment term determined by the amount borrowed:

    • 10 years for borrowers with balances less than $25,000;
    • 15 years for borrowers with balances between $25,000 and $50,000;
    • 20 years for borrowers with balances between $50,000 and $100,000; and
    • 25 years for borrowers with balances greater than $100,000.

    Monthly payments would be fixed for the life of the loan. Borrowers with balances greater than $25,000 who fully repay their loans over the longer repayment period would pay more interest, but their monthly payments would be smaller than if they were in a 10-year standard plan.

    Borrowers would be able to select a new IDR plan, called the Repayment Assistance Plan, which would:

    • Set a minimum monthly payment of $10. All existing IDR plans generally allow for payments of zero for borrowers with low income.
    • Set payments to between 1 percent and 10 percent of a borrower’s total adjusted gross income, depending on the borrower’s income, and reduce payments by $50 per month for every dependent child. Under the current SAVE Plan, borrowers pay between 5 percent and 10 percent of their income above 225 percent of the federal poverty guideline, after accounting for family size.
    • Waive 100 percent of unpaid, accrued interest when a borrower’s calculated payment does not cover accrued interest; the same is true for the current SAVE Plan.
    • Match the monthly amount paid by borrowers up to $50 and apply that match to the outstanding principal balance; the current SAVE Plan has no such match.
    • Forgive any remaining balance after 30 years of repayment. The current SAVE Plan forgives balances after 10 to 25 years of repayment, depending on the loan type and amount borrowed.
    • Require borrowers to remain on the plan until their balance is paid in full, or 30 years, whichever is sooner. Currently, borrowers can switch into other plans.

    Under the legislation, CBO estimates that about 40 percent of the loan volume originated after June 30, 2026, would be repaid through the proposed IDR plan. In contrast, under current law, CBO estimates that roughly 70 percent of loan volume would be repaid under existing IDR plans. Borrowers repaying their loans would pay more, on average, under the IDR plan proposed in the legislation than under current law. For new loans, CBO estimates that implementing the new repayment plans would decrease outlays by $133.6 billion over the 2025-2034 period.

    Borrowers in Repayment.Under subtitle C, borrowers who currently are in any IDR plan would be transferred to a newly proposed IDR plan. Under that plan, payments would be set at 15 percent of a borrower’s discretionary income, with no cap on payment amounts, and borrowers would receive forgiveness of any outstanding debt after 20 years in repayment if they have undergraduate loans only and 25 years if they also have graduate loans. Borrowers could also opt into the new Repayment Assistance Plan (described above) or into a standard repayment plan.

    As required by FCRA, the savings from changes to the costs of existing loans would be recorded in fiscal year 2025. CBO estimates that changes to repayment terms for borrowers currently in repayment would reduce outlays by $162.0 billion in fiscal year 2025.

    Other Changes. Enacting subtitle C also would have other effects:

    • For loans disbursed on or after July 1, 2025, the subtitle would eliminate unemployment and economic hardship deferments and reduce the total period a borrower may be in forbearance. CBO expects borrowers who otherwise would have taken those types of deferments would, under the legislation, enroll in the new IDR plan, begin repaying sooner than under current law, or default. On average, CBO estimates that borrowers would pay less on their loans under the legislation than under current law. CBO estimates that enacting this provision would increase outlays by $340 million over the 2025-2034 period.
    • Loan repayments by new graduate doctors and dentists during residency would not be counted toward the total number of payments needed to qualify for the Public Service Loan Forgiveness Program. The provision also would allow four years of interest-free forbearance for borrowers in medical or dental internships or residencies on loans disbursed on or after July 1, 2025. CBO estimates that implementing this provision would, on net, decrease outlays by $430 million over the 2025-2034 period.
    • Borrowers would be permitted to rehabilitate defaulted loans twice. CBO estimates that implementing this provision would increase outlays by $130 million over the 2025-2034 period.
    • The legislation would directly appropriate $500 million in fiscal year 2025 and in fiscal year 2026 for servicing student loans. CBO estimates that implementing this provision would increase outlays by $1.0 billion over the 2025-2034 period.

    Subtitle D. Pell Grants

    Subtitle D would change eligibility rules for the Federal Pell Grant Program. Although the effective date for most of the subtitle’s provisions is July 1, 2025, CBO expects that date would not provide sufficient time to implement the provisions for the 2025-2026 academic year, which begins on July 1, 2025. We assume for this estimate that those provisions will take effect on July 1, 2026, for the 2026-2027 academic year.

    Pell grant eligibility is determined by the Student Aid Index, a formula that accounts for students’ income and assets and, for dependent students, family income and assets. An SAI is calculated for each student and used to determine their award amount; a higher SAI represents lower financial need. Awards are prorated relative to the definition of full-time enrollment for their school’s curriculum type. Students who qualify for an amount below the maximum, or who do not qualify on the basis of their SAI, may still qualify if their adjusted gross income meets thresholds that are based on the federal poverty guideline.

    Most of the estimates below are based on analyzing a sample of aid applicants and Pell grant recipients that CBO received from the Department of Education. Additional sources of data are discussed with each estimate.

    The costs discussed here are for direct spending outlays only; they involve changes to the mandatory add-on. CBO has not reviewed the legislation for changes in spending subject to appropriation, and estimates of the cost for the discretionary portion of the program are not included.

    CBO estimates that enacting subtitle D would increase direct spending outlays by $2.8 billion over the 2025-2034 period.

    Foreign Income and Federal Pell Grant Eligibility. Subtitle D would amend the eligibility calculation to include foreign income, most of which is excluded from the calculation under current law. That would reduce the award amounts for some recipients with foreign income. CBO estimates that less than 1 percent of Pell grant recipients earn foreign income. On that basis, CBO estimates that enacting this provision would reduce direct spending outlays by $66 million over the 2025-2034 period.

    Change the Definition of Full-Time Enrollment. Subtitle D would increase the number of credits needed to qualify for full-time enrollment from 12 per semester to 30 per year. Under current law, students who are enrolled less than full time receive prorated grants. Raising the number of credits would decrease award amounts for students who currently are enrolled in fewer than 30 credits per year. CBO estimates that under this provision, more than half of students currently enrolled would receive smaller grants. Based on past award increases, National Student Clearinghouse data on time to completion, and existing financial incentives for early graduation, CBO estimates that about one-fifth of expected grant recipients would enroll in additional credits to increase their award amounts. On that basis, CBO estimates that enacting this provision would reduce direct spending outlays by $7.1 billion over the 2025‑2034 period.

    Eliminate Eligibility for Students With a High SAI. Subtitle D would eliminate eligibility for students whose SAI is double the amount for the Pell grant maximum award. CBO estimates that less than 1 percent of Pell grant recipients meet or exceed that threshold, and those who do generally receive the minimum award. On that basis, CBO estimates that enacting this provision would reduce direct spending outlays by $78 million over the 2025‑2034 period.

    Eliminate Eligibility for Students Enrolled Less Than Half Time. Subtitle D would require a student to be enrolled half time, that is, for at least six credits per semester, to receive a grant. Program data indicate that in recent academic years roughly 10 percent of recipients were enrolled for less than half time. Based on past increases under the program and data from the National Student Clearinghouse on time to completion, CBO expects that about one-third of the recipients who would lose their award under this provision would enroll in additional credits to avoid doing so. CBO estimates that enacting this provision would reduce direct spending outlays by $687 million over the 2025-2034 period.

    Workforce Pell Grants. Subtitle D would extend eligibility for Pell grants to students enrolled in workforce programs that can be completed in 150 to 600 clock hours, or an equivalent number of credit hours, provided the program meets standards for certification, completion, and after-graduation earnings. Under current law, students enrolled in programs requiring fewer than 600 clock hours are ineligible for Pell grants.

    Using data from the Department of Education, statistics from the American Association of Community Colleges, and published reports, CBO estimates that, under the legislation, by 2034 about 100,000 new recipients each year would receive Workforce Pell Grants of about $2,200 each (about 20 percent of that amount would come from mandatory funds). On that basis, CBO estimates that enacting the provision would increase the cost of the mandatory add-on by $298 million over the 2025-2034 period.

    To be eligible for Pell grant funds, postsecondary programs would need to demonstrate job placement and completion rates of at least 70 percent. Their tuition and fees must not exceed the difference between the median earnings of students who complete the program and 150 percent of the federal poverty guideline.

    CBO expects that fewer than half of the current short-term programs at institutions that already receive financial aid under title IV of the Higher Education Act would become newly eligible under the legislation. However, using information from community colleges and research on postsecondary education, CBO expects that many of the students already receive Pell grants because they are enrolled in short-term programs that are “stacked” within longer-term programs that are eligible for Pell grant funding. As a result, under current law, those students can receive Pell grants even if they do not complete the longer-term program.

    In addition, many short-term programs that do not currently receive federal financial aid funding, particularly those in the proprietary sector, would not participate in the Pell Grant Program under the legislation. Those institutions would be excluded either because they could not meet the requirements in the legislation or because they would choose not to meet the additional requirements for participation in federal student aid programs.

    Pell Shortfall. Subtitle D would directly appropriate additional mandatory funds to support the portion of Pell grants funded mostly through annual discretionary appropriations: $3.2 billion in 2026, $4.8 billion in 2027, and $2.5 billion in 2028. Enacting the provision would increase direct spending outlays by $10.5 billion over the 2025-2034 period, CBO estimates.

    Subtitle E. Accountability

    Under the legislation, postsecondary institutions could be required to make annual payments, called risk-sharing payments, in order to participate in the federal student loan program. Those payments would be the main source of funding for the Promoting Real Opportunities to Maximize Investments and Savings in Education (PROMISE) grants, which would be made to eligible postsecondary education institutions to help improve affordability and promote success for students.

    CBO estimated the amounts in risk-sharing payments on a cash basis rather than using FCRA procedures because those annual payments are based on cohorts of loans and are not tied directly to, or made on behalf of, any individual loan. The legislation defines loan cohorts as groups of loans to borrowers who exit a program in the same year. CBO estimated the effects of those provisions as if all other provisions in the legislation were enacted simultaneously. For example, the estimate for the amount of risk-sharing payments incorporates the assumptions that borrowers would no longer be eligible for the current SAVE Plan, that grad PLUS loans would no longer be available, and that new loan limits would be in place.

    CBO estimates that enacting subtitle E would reduce direct spending outlays by $6.2 billion over the 2025‑2034 period.

    Risk-Sharing Payments. The legislation would require some institutions to make annual payments to the Department of Education as a condition for participating in the student loan program. Those payments would be recorded as offsetting receipts—that is, as reductions in direct spending. Payments would be based on a formula that considers the amount of loan payments in a cohort that are waived, matched, or forgiven in the new IDR plan or that borrowers fail to make in a timely manner; the total cost of a program for borrowers who complete that program; and borrowers’ expected future earnings.

    CBO calculated risk-sharing payments based on our estimates of repayments under the legislation’s proposed Repayment Assistance Plan, information from the College Scorecard database (which gathers data on institutional costs, graduation and employment rates, and student loan borrowing), and the Integrated Postsecondary Education Data System. CBO also analyzed delinquency and default rates using data from NSLDS.

    CBO anticipates that the first risk-sharing payments would be made by institutions late in fiscal year 2028, after the Department of Education issues new rules, and that the department would apply the requirements prospectively on loans made beginning in the 2027-2028 academic year. We expect that initially, risk-sharing payments would be small but would increase as more borrowers entered repayment on loans originated after June 30, 2027. CBO estimates that by 2034, risk-sharing payments would be $1.3 billion and would continue to increase after that year.

    CBO estimates that enacting this provision would reduce outlays by $5.3 billion over the 2025-2034 period.

    Reduction in Institutional Participation in Federal Student Aid Programs.Given the high cost of risk-sharing payments to institutions and the considerable uncertainty about that cost over the lifetime of any given loan, CBO expects that some institutions would take action to avoid making those payments: Some would choose not to participate in the federal student loan program, others would close certain institutional programs, and still others would close altogether. Based on CBO’s analysis of calculated risk-sharing payments, information from associations of schools and from people with knowledge of postsecondary financial aid programs, we estimate that enacting this provision would reduce projected loan volume, after all other policies in the legislation, by roughly 20 percent.

    By 2028, CBO estimates that, after incorporating all of the provisions of the legislation, 1 dollar of student loan volume would cost the federal government, on average, about 3 cents. On that basis, CBO estimates that the reduction in loan volume would reduce outlays by $3.6 billion over the 2025‑2034 period.

    CBO expects that decisions by institutions to avoid risk-sharing payments also would affect federal spending for the Pell grant mandatory add-on. In general, institutions that leave the federal student loan program would be expected to continue to participate in the Pell Grant Program. However, based on the literature included as part of the Department of Education’s rulemaking on gainful employment and financial transparency (see “Subtitle F, Regulatory Relief” below for more information), CBO expects that some students enrolled in programs or schools that close as a result of the legislation’s risk-sharing requirements would not reenroll in other programs. Thus, CBO estimates that enacting the risk-sharing provision would reduce direct spending outlays for the Pell grant mandatory add-on by $397 million over the 2025‑2034 period.

    PROMISE Grants. The legislation would institute PROMISE grants, funded by institutional risk-sharing payments. Institutions would be required to meet certain requirements to be eligible for the grants, including guaranteeing a maximum total price charged to a student for a given program.

    Under the grant formula, an eligible institution could receive up to $5,000 for each student receiving federal financial aid each year, depending on the availability of funds. Along with additional criteria, the formula compares students’ earnings after completion of a program with the cost of tuition.

    CBO expects that PROMISE grants, which would be classified as direct spending, would be awarded as funds become available. Using information from the College Scorecard database and the Integrated Postsecondary Education Data System and considering estimated risk-sharing payments, CBO estimates that PROMISE grants would increase outlays by $3.0 billion over the 2025-2034 period.

    Return of Title IV Funds for Student Loans and the Pell Grant Mandatory Add-On. The legislation would allow the Department of Education to reallocate federal student aid that is returned to the government under title IV of the Higher Education Act to fund PROMISE grants. CBO estimates that enacting this provision would increase direct spending for student loans because it would change the underlying cost of those loans. Funding PROMISE grants with returned funds from Pell grants also would increase direct spending because the mandatory add-on for Pell grants is not subject to appropriation. CBO estimates that using those returned funds for PROMISE grants would increase direct spending outlays by $111 million over the 2025-2034 period.

    Subtitle F. Regulatory Relief

    The legislation would repeal several rules and regulations affecting institutional eligibility for federal student aid, and the terms under which a student loan borrower could receive forgiveness.

    CBO estimates that enacting subtitle F would reduce direct spending outlays by $9.0 billion over the 2025‑2034 period.

    Repeal the 90/10 Rule. The legislation would repeal the requirement that for-profit institutions receive no more than 90 percent of their revenue from federal financial aid, including veterans’ education benefits. CBO anticipates that repealing the rule would allow schools whose revenue comes primarily from federal sources to expand enrollment and that the schools closest to the 90 percent threshold would be the most likely to do so. CBO estimates that enacting this provision would increase direct spending outlays by about $1.6 billion over the 2025-2034 period: $1.3 billion for increased student loan volume, $297 million for the Pell grant mandatory add-on, and $25 million for veterans’ education benefits.

    Repeal the Gainful Employment Rule. The legislation strikes all references to “gainful employment” from the Higher Education Act. CBO expects that the Department of Education would implement that change by repealing the regulations related to gainful employment. Those regulations establish a debt-to-earnings ratio and an earnings premium test that for-profit institutions, and certain non-degree-granting programs at two-year institutions, would need to meet for the programs to remain eligible for federal student aid. Based on a literature review, CBO estimates that repealing the rules would increase both student borrowing and the number of Pell grant recipients by about 2 percent. On that basis, CBO estimates that enacting the provision would increase direct spending outlays by about $6 billion over the 2025‑2034 period: $5.1 billion for student loans and $918 million for the Pell grant mandatory add-on.

    Repeal the Closed-Schools Discharges Rule. The legislation would repeal a rule that established an automatic process for discharging loans made to borrowers who attended schools that closed, thus increasing the likelihood of loan discharge for those borrowers. Using information from the Department of Education, CBO estimates that repealing the rule would reduce outlays by $5.2 billion over the 2025-2034 period.

    Repeal the Borrower Defense to Repayment Rule. The legislation would repeal a rule that made it easier for borrowers’ loans to be discharged as a result of a school’s misconduct, including, for example, misrepresentation of student outcomes. Based on an analysis of loan volume at schools that were or are under investigation for issues that could fall under that rule, and using data from the Department of Education, CBO estimates that enacting the change would reduce outlays by $11.5 billion over the 2025-2034 period.

    Subtitle G. Limitation on Authority

    Subtitle G would limit the authority of the Department of Education to issue regulations that would increase the cost of federal student loans or that would have economically significant effects (that is, that would have an annual effect on the economy of $100 million or more or that would adversely affect the economy in a material way). CBO’s baseline includes costs that reflect the possibility of future administrative actions that would increase the cost to the government of federal student loans.

    CBO estimates that enacting subtitle G would decrease outlays for student loans by $31.8 billion over the 2025‑2034 period.

    Interactions Among Provisions

    Most provisions discussed in this document were estimated relative to current law. The effects on direct spending of simultaneously enacting all of the provisions in the legislation would differ from the sum of effects from enacting each provision separately relative to CBO’s baseline.

    The estimates for provisions to which that does not apply concern the risk-sharing payments and PROMISE grants, which were estimated relative to CBO’s baseline as adjusted to include the effects of all other policies in the legislation. Those estimates contain some interactions not shown in the “Interactions” row in Chief, Finance, Housing, and Education Cost Estimates Unit

    Almost million more pupils get access to mental health support

    School mental health support rollout to reach up to 900,000 more pupils this year, covering six in ten children across the country.

    Almost one million more young people will have access to mental health support in school this year, as the government gets on with delivering its manifesto promise for a national rollout taking a huge step in fixing the inherited challenges facing our children.   

    Under government plans, all pupils will have access to mental health support in school by 2029/30, delivering on its Plan for Change to improve children’s life chances and tackle the root causes of poor attendance and behaviour.  

    The support teams are made up of specialists who offer a range of help to identify and tackle issues early on, from group sessions to build children’s resilience to one-to-ones helping to manage anxiety – not only tackling the crisis of poor mental health among young people, but also driving up school attendance.

    They work directly with school and college staff alongside NHS services to provide professional advice, easing the pressure on school staff and allowing them to help young people get the right support and stay in education. New research has shown a direct link between the severity of children’s mental health problems and their likelihood to miss school. 

    The new investment means six in ten pupils will have access to a mental health support team by March 2026, with the rollout prioritised based on NHS identification of local need and reaching the most vulnerable children first. 

    It comes as government unveils a new programme to provide intensive support for 500 schools with significant attendance and behaviour challenges. New attendance and behaviour hubs, built around 90 schools with a track record of improving attendance and behaviour standards, will directly target the schools with the highest need as well as providing wider support for a further 4,500 in all corners of the country.

    These will work alongside our new attendance and behaviour ambassadors who will be the link between schools and the government, identifying challenges and working jointly toward solutions.

    This move builds on the progress this government has already made to turn the tide on school attendance, with green shoots already being seen with over three million more days in school than last year. 

    Education Secretary Bridget Phillipson said: 

    We inherited a system full of challenges and breaking the vicious cycle of poor mental health, low attendance and bad behaviour among children and young people is the most urgent one facing our schools – and this government is already turning the tide.

    Expanding mental health support for young people is one of the single biggest steps we can take to improve children’s life chances, make sure all pupils are getting the very most out of school and deliver excellence for every child. 

    Taken alongside new intensive support for schools that are struggling, our free breakfast clubs for millions of children and our wider work to drive up school attendance, this government will continue using all available levers to break the destructive link between background and success and deliver on our Plan for Change.

    Health and Social Care Secretary Wes Streeting said: 

    Facing mental health problems when you’re young can hold you back in school, damage your potential and leave you with lifelong consequences. It’s devastating and it’s got to change.

    That’s why this government is bringing in vital services to schools, so they can intervene early, support pupils, and help prevent conditions from becoming severe.

    Backed by an extra £680 million in government funding this year, we are transforming mental health services for children – hiring more staff, delivering more talking therapies, and getting waiting lists down through our Plan for Change – so children can have the best possible start in life.

    Pepe Di’Iasio, General Secretary of the Association of School and College Leaders, said:

    There has been a growing crisis in the mental health of children and young people in recent years, with too many people having to wait too long to get the help they need. 

    It is reassuring that the government recognises this problem, as well as the impact it is having in schools, and that mental health support will be available to significantly more pupils over the next year.

    This is a welcome step towards reaching a point where all children and young people can access this specialist support if required.

    Poor attendance has a direct impact on a pupils’ attainment, future earnings and life chances – with persistently absent pupils in secondary school earning £10,000 less at age 28 compared to pupils with strong attendance. 

    Persistent absence, suspensions and exclusion rates lay bare the broken system that this government has inherited, and while there are green shoots in the data, the government is determined to go further and faster, joining up mental health, wellbeing, attendance and behaviour to drive change for young people.   

    Mental Health Support Teams currently reach around seven in ten secondary school pupils. 

    The teams are made up of specialists who intervene early with children with mild to moderate mental health issues, empowering them to work through challenges in a calm and supportive learning environment. They also provide timely advice to school and college staff to develop a whole school approach to mental health and liaise with external specialist services such as the NHS, to help young people get the right support and stay in education. 

    Mr Gary Lloyd, Head Teacher at The Academy of St Nicholas, Liverpool, which has had access to a mental health support team for a year, said: 

    Having a trained, known, trusted professional working with the academy has made such a difference in supporting staff and students.  

    They support our wider inclusion strategies – often triaging and getting support to our young people much faster – which is impacting positively on attendance and general happiness within school.

    More widely, the government is recruiting an additional 8,500 mental health workers across children and adult mental health services to reduce delays and provide faster treatment. This will work alongside the further 6,500 teachers so every classroom has a brilliant teacher, to ensure that every child is supported in and outside of school.  

    The government is also taking wider action to drive up standards in schools across the country with innovative RISE teams, to ensure every child has a great education. They will work alongside the attendance and behaviour hubs to drive up attendance in underperforming schools so every child can achieve and thrive.

    “We are writing today to express our serious concerns regarding efforts by the Trump administration and the potential of cuts proposed in your budget resolution to debilitate our nation’s beloved nutrition programs for seniors, taking hot meals, sustenance, and social interaction away from our seniors to fund tax cuts for billionaires,” the lawmakers wrote to Senate Republicans. “These attacks are multifaceted and will deeply hurt all aspects of senior nutrition services, from funding to program delivery. The cuts will exacerbate hunger, poor health, and social isolation, and our nation’s seniors will be gravely harmed by these decisions.”
    If implemented, the Republicans’ proposed cuts will take hot meals, nutritious food, and social interaction away from millions of older adults nationwide, jeopardizing their health and quality of life.

    Cuts to the Supplemental Nutrition Assistance Program, which is the largest food assistance program in the country; 
    The dismantling of the Administration for Community Living, which administers the Older Americans Act’s nutrition programs; 
    The elimination of the Social Services Block Grant, which provides critical funding for senior nutrition programs;
    Cuts to Medicaid, which funds meal-delivery programs through Section 1115 waivers for home-bound older adults and people with disabilities; and
    Attacks on the Social Security Administration, which ensures accurate and timely payment of Social Security benefits so older adults and people with disabilities can put food on the table.
    The letter was led by U.S. Senator Kirsten Gillibrand, D-N.Y. Along with Wyden and Merkley, the letter was also signed by U.S. Senators Bernie Sanders, I-Vt., Amy Klobuchar, D-Minn., and Senate Minority Leader Chuck Schumer, D-N.Y. The senators wrote in their capacities as ranking members of the Senate Committees on Aging; Finance; Budget; Health, Education, Labor, and Pensions; Agriculture, Nutrition, and Forestry; and as Senate Democratic Leader, respectively.

    Survey launched to inform NHS dental contract reform

    Dentists in England encouraged to take part to inform government plans to improve NHS dentistry

    • Dentists nationwide encouraged to take part in survey on costs of running dental practices
    • Findings will support government’s plans to reform dental contract by giving a more accurate picture of what is driving up dental costs
    • Research is part of mission to improve access to dental care for patients through government’s Plan for Change

    Dentists across England are being urged to take part in a new nationwide survey to help inform the government’s long-term dental reform programme.

    The survey will gather information on the costs and pressures involved in running a dental practice.

    The research is part of the government’s wider plans to reform the dental contract in England, providing better access to care for patients by making NHS work more appealing to dentists.

    Health Minister Stephen Kinnock said:

    We are working to fix an NHS dentistry sector left broken by years of neglect.

    We have already rolled out an extra 700,000 urgent dentistry appointments and introduced a supervised toothbrushing programme to prevent tooth decay in young children in the most deprived communities. 

    More work is needed, but to find the right solution we must make sure we are clear about the problem. Through this survey, we will gain a better understanding of the pressures faced by the sector so we can fix them and deliver better care for patients through our Plan for Change.

    Results of the survey will support the development of the government’s dental reform programme and the annual pay review process conducted by the independent Review Body on Doctors’ and Dentists’ Remuneration (DDRB).

    It forms part of the government’s Plan for Change to improve NHS dental services, addressing challenges that have left many patients struggling to access care, amid reports that some have undertaken DIY dentistry.

    The government has started on its manifesto commitment to roll out extra urgent dental care appointments across the country.

    It is particularly targeting areas of dental deserts, where patients have struggled to get appointments, and has rolled out a national supervised toothbrushing programme for 3 to 5 year olds in early years settings – including nurseries and primary schools.

    Practice owners who complete the anonymous survey can also register their interest in participating in follow-up interviews to provide more detailed insights into the financial challenges they face.

    The survey is open to all dental practices across England until 16 June 2025.

Kennedy Jr. to discuss the reorganization of HHS programs that assist vulnerable Alaskans including Low-Income Home Energy Assistance Program (LIHEAP), National Institute for Occupational Safety and Health (NIOSH), Head Start, and community-based programs that support survivors of domestic violence.

    Murkowski: Thank you, Mr. Chairman. Mr. Secretary, welcome, good to see you. I want to talk a little bit about the HHS reorganization on some of the programs that impact Alaska’s most vulnerable populations. I sent you a note letting you know that just after this hearing, I’m going to be chairing a Senate Committee on Indian Affairs [hearing] specifically examining HHS Tribal programs that are outside of IHS [Indian Health Service]. I really thank you for your early efforts to exempt IHS healthcare providers from the RIFs [reductions in force], that was very important.
    But, I’ve also heard concerns from Tribal leaders on the impacts of RIFs to key HHS programs serving their communities. So, I know you’re going to have some of your folks tuning in on that, and I really appreciate that. But some of the other reductions that we’re looking at within your budget do have significant consequences to a state like mine.
    One is the LIHEAP program, the low-income energy assistance. For us it’s not a budget line item. You’ve been to Alaska. You know that the temperatures there can get really, really tough. [LIHEAP] keeps people from freezing to death in their homes.
    Another program is NIOSH, and I know that HHS had rescinded a number of those employees, that was great news. But employees that received RIF notices for the program were not rescinded in the NIOSH center for Marine Safety and Health Studies. So, this is a big deal for our commercial fishing safety. It could effectively leave our fishing fleet out of compliance with Coast Guard safety [regulations], so we’re watching that very, very carefully. And then again, shared focus here on making sure that our children are as healthy as they possibly can be. I want to look to ways that we can strengthen and not eliminate the Head Start program.
    Kennedy: You’re talking about the NIOSH program? You should talk to me about that. As you know, that’s something that I’m deeply concerned with, with the commercial fisheries. So, we should talk about it. Let’s work for the solution.
    Murkowski: Got it. I am with you right there.
    Let me ask about domestic violence and sexual assault funding. Right now, I’m talking and I’m receiving a lot of incoming from our community-based domestic and sexual violence program operators. They’re really concerned about the delayed release of FY 25 funding, the absence of notices of funding opportunities, as well as proposed cuts or consolidations that might threaten the Office of Family Violence Prevention and CDC’s Division of Violence Prevention. So, you’ve got some programs there that are really foundational to domestic and sexual violence. They’ve been reauthorized with bipartisan support.
    So, I’m going to enter into the record a letter from the National Task Force [to end Sexual and Domestic Violence], and it was sent to you yesterday, just urging the communication of concrete plans for releasing some of these funds. I want to raise that to your level, but I want to make sure that we’re sending the right signal to so many who are just really on the edge with, again, these community-based services that are helping the most vulnerable of the most vulnerable. So, we’ve got the funding that’s out there. It’s just delayed. We need help releasing that.

  • MIL-OSI New Zealand: NZ to subject Pandemic Treaty to full National Interest Test

    The Government will subject a draft Pandemic Treaty to a full National Interest Test before deciding whether or not New Zealand should sign up to it, Foreign Minister Winston Peters and Health Minister Simeon Brown say. 

    The draft Treaty will be discussed at the annual meeting of the World Health Assembly in Geneva from next week 

    “Decisions about how New Zealand responds to any health emergency are made in Wellington, not Geneva, and we are determined to preserve our sovereign decision-making ability,” Mr Peters says.

    “While negotiations on the draft Treaty have been completed, there are a number of steps remaining – which are likely to take some years – before the New Zealand Government will take a decision on whether or not we should sign up.”

    Mr Brown noted that, after three years of negotiation, the World Health Assembly next week would have the Pandemic Treaty presented to it for adoption.

    “New Zealand supports the aim of strengthening global health systems and to improve pandemic preparedness and response,” Mr Brown says.

    “This will also help New Zealand support our Pacific partners to better manage future pandemic responses in our region.”

    Mr Peters says that it is very important to New Zealand that its sovereign decision making on health is protected. 

    “Any Treaty needs to confirm the sovereignty of countries to address public health matters within their borders and gives no power to the WHO or any other international body to direct, order, or change national laws or policies, or to direct countries to take specific actions, such as ban or accept travellers, impose vaccination mandates, or implement lockdowns.”

    “For these reasons, any future decision on whether or not to sign and ratify the Pandemic Treaty will be informed by a full National Interest Test.”

    Kopu Road between Kopu and Thames has reopened, after being closed earlier this morning when a person was found with what appeared to be serious injuries.

    Police have now established that the person had suffered a medical event.

    The person has been airlifted to Waikato Hospital and the road has reopened.

    [. In addition to the work in the assembly, this session saw the government advocate fiercely for a strong and sovereign Alberta within a united Canada, build and strengthen relationships with trade partners, and defend Alberta’s economy and Albertan jobs.

    In the face of global trade tensions and market uncertainty, the rising cost of living remains one of the largest challenges facing Albertans. This spring, Alberta’s government took action to ease that burden. Budget 2025 delivered the promised income tax cut, saving families up to $1,500 per year. The Automobile Insurance Act was passed to enable better, faster, cheaper auto insurance for Albertans, and we passed legislation to expand energy options by enabling hydrogen blending and making critical reforms to ensure Albertans have access to affordable, reliable utilities when they need them most.

    “Every piece of legislation our government brought forward this session was driven by one goal: to make life better for Albertans. I’m proud to be part of a team that meets the challenges Albertans are facing today and positions our province for long-term success.”

    Alberta’s government also passed legislation to deliver on its mandate to restore health and safety for families and communities. The Compassionate Intervention Act introduced a new approach to addressing the addiction crisisadding another tool to the Alberta Recovery Model and giving Albertans struggling with severe addiction the opportunity to rebuild their lives and reconnect with their family, community and culture. Legislation was also passed that implements lessons learned during previous emergency responses and empowers municipalities through expanded options for local policing.

    This session Alberta’s government passed 19 bills, fulfilled multiple platform commitments and delivered on the strong mandate received from Albertans two years ago.

    • The Agricultural Operation Practices Amendment Act provides clarity for the emerging biogas industry, spurring job-creating investment in rural Alberta.
    • The Critical Infrastructure Defence Amendment Act protects essential infrastructure and supports the government’s work under the Alberta Sovereignty Within a United Canada Act.
    • The Education Amendment Act reflects changes in the education landscape, strengthening democratic accountability in school boards, and increasing clarity and efficiency in the teacher discipline process.
    • The Election Statutes Amendment Act protects democracy, delivers fair and open elections and restores confidence in every vote cast.
    • The Health Statutes Amendment Act continues the work to refocus the healthcare system, ensuring patients receive the care they need, when and where they need it.
    • The Municipal Affairs Statutes Amendment Act strengthens local governance and collaboration, streamlines processes and bolsters protections for new home builders and buyers.
    • The Professional Governance Act ensures Alberta has a modern, uniform governance framework for professional regulatory organizations.
    • The Wildlife Amendment Act aligns the Wildlife Act with current knowledge and best practices, supporting enhanced opportunities for hunting and trapping, reducing human-wildlife conflicts and streamlining enforcement approaches.

  • MIL-OSI Canada: Detect early, stop cancer

    Half of all people living in Alberta will have cancer during their lifetime. They deserve high-quality cancer care, including early detection. That’s why Alberta’s government is extending the Alberta Lung Cancer Screening Program and is providing grants to four organizations to improve access to cancer screening – particularly in rural, remote and Indigenous communities.

    Launched as a pilot in 2022, the Alberta Lung Cancer Screening Program was designed to assess whether a provincially coordinated approach could improve lung cancer outcomes through earlier detection and timely treatment. After a promising launch, it is being extended with a $2-million investment and an additional $500,000 from the Alberta Cancer Foundation. This funding will allow the pilot to continue offering screenings to eligible individuals for an additional year while plans are finalized to transition the program to a permanent, provincewide program. 

    “We’re making progress, with more people in Alberta surviving lung cancer than ever before, but more work remains. That’s why we’re funding the Alberta Lung Cancer Screening Program to support early detection and improved patient outcomes.”

    Treatment for early-stage lung cancer is significantly more effective, less burdensome for patients and their families, and more cost-effective for the health care system. Since its launch, the Alberta Lung Cancer Screening Program has screened more than 3,800 eligible people in Alberta, with almost 90 per cent of detected lung cancers identified at an early stage.

    “Late-stage lung cancer is much harder to treat or cure because the cancer has already spread to other parts of the body or there’s too much cancer in the lungs. Screening with low-dose CT scans can detect lung cancer earlier, before someone has symptoms.”

    More funding available for screening, prevention

    Detecting cancer early is crucial to saving lives, and Alberta’s government continues to make early detection more accessible and easier to navigate.

    The Cancer Research in Screening and Prevention Program is providing about $3 million to support cancer prevention and screening initiatives to improve health outcomes for people living in Alberta, including research, education, marketing and public policy development. Eligible Alberta-based health agencies and organizations, post-secondary institutions, non-profits, First Nations and Métis communities, as well as municipalities, can apply for funding under the program.

    Four organizations received funding for their projects in 2024:

    • Alberta Health Services – Optimizing screening for subsequent primary cancers in recipients of hematopoietic cell transplantation – $343,518
    • Lakeland Métis Nation Association – Lakeland Métis Nation cancer screening awareness program – $600,000
    • Siksika Health Services – Siksika Nation cancer screening and prevention – $997,850
    • University of Alberta – A Phase 3 randomized trial of prostate cancer screening using high resolution micro-ultrasound versus MRI – $987,000

    TUV representatives from all levels of government were pleased to attend this year’s Balmoral Show. Under blue skies and amidst strong crowds, our team engaged with many members of the farming community to listen, discuss and stand alongside them on the pressing issues facing agriculture today.

    Across countless conversations, the same key concerns emerged:
    • Labour’s proposed plans to extend Inheritance Tax – a direct threat to family farms.
    • Stormont’s new Nutrient Action Plan – viewed by many as unworkable and deeply unfair to local producers.
    • The ongoing crisis of Bovine TB – a long-standing issue that still lacks effective resolution.

    Many livestock and poultry breeders raised concerns over continuing difficulties in moving animals across the Irish Sea Border — whether for shows, sales, or the introduction of new bloodlines to pedigree stock. The end of the grace period for veterinary medicines from GB to Northern Ireland in December 2025 also remains a critical worry for many.

    Added to this are growing national and international pressures:
    • The trade deal to import beef from America.
    • GB’s ongoing challenges with Bluetongue.
    • Europe’s outbreak of Foot and Mouth Disease.

    These are not abstract concerns — they are matters which threaten livelihoods, food security, and the future of rural life.

    New clinical guidance will offer best practices for recognizing and supporting people at risk for suicide, helping more people get the right care and saving more lives.

    “Every life lost to suicide is a profound tragedy,” said Josie Osborne, Minister of Health. “This new framework represents a critical step in ensuring that individuals experiencing suicidal thoughts can access the support they need to move toward hope and healing. It sets a clear path for how we will care for and support those most at risk.”

    In partnership with the Province, the Canadian Mental Health Association, BC Division (CMHA BC) led the development of a suicide-risk-reduction framework to support health-care organizations in improving care provided to people at risk for suicide. It will apply to patients who are 18 and older and are accessing mental-health or substance use care in hospital emergency departments, acute psychiatry or medical inpatient units or outpatient mental-health services.

    “People experiencing a mental-health crisis need to be met with compassion and person-centred care,” said Amna Shah, parliamentary secretary for mental health and addictions. “When someone is experiencing thoughts of suicide, it is especially important that they get timely supports for as long as they need. This framework will support our health-care facilities in offering best practice guidance so clinicians can help more people with the right care.”

    Through an early, consistent, and systematic process, the framework offers guidance to enhance the detection of individuals at risk for suicide. It also provides suggestions that can help eliminate bias and barriers to care, including stigma and experiences of discrimination or Indigenous-specific racism.

    It also includes best practices to maximize the patient’s safety during and after their care. With a tailored care plan, evidence-based treatment, effective transition to community care, and follow up and monitoring post-discharge, better outcomes can be achieved.

    “When someone is struggling with thoughts of suicide, asking compassionate questions and truly listening can make all the difference,” said Jonny Morris, CEO, CMHA BC. “This framework helps health-care teams have these vital conversations, understand each person’s unique story, and support informed decisions about care. We’re deeply grateful to the Province of B.C., our partners, and especially the courageous and wise individuals whose lived experiences shaped this important work.”

    Building on best practices in Canadian and international jurisdictions, the framework was created with input from people with lived and living experience with mental-health crises. Indigenous cultural safety is embedded throughout the framework, as Indigenous Peoples disproportionately experience poorer health outcomes within the health-care system.

    In addition to prioritizing cultural safety, the framework is based on a foundation of patient and family engagement, trauma-informed care and close collaboration with community care providers, which can help ensure the continuity of care.

    “The release of the suicide-risk reduction framework is an important step for British Columbia, and I want to thank our partners for their collaboration,” said Lesley Lutes, professor, director of the Centre for Obesity and Well-Being Research Excellence, department of psychology, UBC Okanagan; and director of advocacy, BC Psychological Association. “When we treat mental health with the same level of rigour and evidence-based interventions as we do with physical health, we save lives.”

    This work is part of the Province’s efforts to build up the entire continuum of mental-health and substance-use care for people to get the right support for them. This includes increasing early intervention and prevention, adding and expanding treatment and recovery services, building complex care housing, adding overdose prevention services and more.

Castor and Soto’s call for a Medicaid fraud investigation comes on the heels of the debate in the U.S. House Energy and Commerce Committee over the future of Medicaid and House Republicans’ cruel proposal to kick millions of Americans off Medicaid to pay for tax breaks for the wealthiest Americans. Republicans repeatedly claimed during the marathon Energy and Commerce debate that they were concerned about waste, fraud and abuse in Medicaid. Castor and Soto now point to a concrete example of potential fraud and abuse, while urging an immediate investigation into this inappropriate diversion of taxpayer funds to an unrelated political action committee. 

    “As members of the U.S. House Committee that provides oversight of Medicaid, I can assure you that Congress is very focused on waste, fraud and abuse of Medicaid dollars. Any unlawful diversion of Medicaid dollars in Florida means that the state is less able to provide services to our neighbors who rely on Medicaid and the providers who serve them,” the lawmakers wrote.

    The lawmakers continued, “The diversion of Medicaid dollars requires immediate investigation. These are proceeds that rightfully belong to state taxpayers to serve the citizens who rely on Medicaid, including children, pregnant women, neighbors with disabilities and those served by long-term care.”

    Castor and Soto serve on the House Energy and Commerce Committee, which has jurisdiction over Medicaid, and advocated for families and providers during the 26-hour Energy and Commerce Committee markup of the House Republicans’ cruel proposal to slash Medicaid to pay for tax breaks for the wealthiest Americans, which concluded yesterday.

    RE: Urge Investigation into Unlawful Diversion of Medicaid Funds in Florida 

    Dear Acting Inspector General Hodgkins and Administrator Oz:

    A recent bipartisan investigation by the Florida Legislature and press reports have uncovered that proceeds from a legal settlement between the State of Florida and Florida’s largest Medicaid managed care operator, Centene, were inappropriately diverted to unrelated political committees. Federal law requires that Medicaid proceeds be used solely for health services authorized by law and for the benefit of those served by Medicaid. Therefore, we respectfully request that you investigate the potential unlawful diversion of Medicaid funds by the Florida Agency for Health Care Administration (AHCA), Hope Florida Foundation and Centene. Medicaid is a federal/state partnership, and the federal government may be entitled to recoup funds from the legal settlement and improperly diverted funds as well.

    Hope Florida was established in 2021 as a referral program, operated by state employees, to direct Floridians to businesses, faith-based organizations and nonprofits for housing and social services instead of to government agencies. The Hope Florida Foundation is Hope Florida’s nonprofit arm and is subject to spending limits on lobbying and campaigns. According to its website, Hope Florida “firmly believe(s) that more government is not always the best solution to the problem. Instead, government is utilized as a meaningful connection point and then gets out of the way.” Many Florida state agencies prominently display links on their homepage to Hope Florida, directing individuals to a Hope Navigator instead of contacting a state agency, including the websites of Florida Department of Children and Families, Florida Department of Juvenile Justice, Florida Department of Veterans Affairs and Florida Department of Elder Affairs. AHCA administers Florida’s Medicaid program.

    On September 27, 2024, the State of Florida reached a settlement agreement with Centene relating to the overbilling of taxpayers by over $67 million. The agreement directed Centene to pay $10 million to the Hope Florida Foundation through a wire transfer and pay the remaining $57 million to AHCA. The settlement also stated that “AHCA desires an expanded role for Hope Florida in the Florida Medicaid program.”  

    On October 16, two days after receiving the $10 million wire transfer, the Hope Florida Foundation wired $5 million to Secure Florida’s Future, a 501(c)4 nonprofit that proposed spending the ‘grant’ on a “long-term, targeted business partner recruitment strategy and public awareness campaign.” 

    On October 17, Secure Florida’s Future donated $2 million to Keep Florida Clean Inc., a Political Action Committee (PAC) controlled by Governor DeSantis’s then-chief of staff James Uthmeier that was created to campaign against Amendment 3, a ballot initiative to legalize recreational marijuana in Florida. Governor DeSantis strongly opposed Amendment 3. Days later, Secure Florida’s Future sent Keep Florida Clean Inc. an additional $1.75 million. 

    On October 22, the Hope Florida Foundation wired $5 million to the 501(c)4 nonprofit Save Our Society from Drugs that proposed spending the ‘grant’ on “developing and implementing strategies that directly address the substance use crisis facing our communities.” 

    On October 23, the next day, Save Our Society from Drugs donated $1.6 million to Keep Florida Clean Inc. Over the coming days, Save Our Society from Drugs donated an additional $3.15 million to Keep Florida Clean Inc. 

    While there are limited financial disclosure requirements associated with 501(c)4 organizations, records appear to show that a total of $8.5 million from the Centene settlement with AHCA went from the Hope Florida Foundation to the Amendment 3-focused Keep Florida Clean, Inc. PAC, the same PAC that also donated funding to the Republican Party of Florida and the Florida Freedom Fund. 

    The transfer of Medicaid dollars to a charitable committee and then political committees appears to run afoul of federal law, including 18 U.S.C. 1347 (to knowingly execute or attempt a scheme to defraud a health care benefit program or obtain money from it) and 18 U.S.C. 371 (for two or more people to agree to defraud the United States.), and may implicate other relevant statutes and regulations. As members of the U.S. House Energy and Commerce Committee that provides oversight of Medicaid, we can assure you that Congress is very focused on waste, fraud and abuse of Medicaid dollars. Any unlawful diversion of Medicaid dollars in Florida means that the state is less able to provide services to our neighbors who rely on Medicaid and support the providers who serve them.

    Hope Florida had raised only about $2 million during its three years of existence, but in one fell swoop, received $10 million from a Medicaid settlement, which was immediately funneled through other nonprofits to a PAC directed by the Governor’s Chief of Staff. The Florida House of Representatives initiated an investigation into what State Representative Alex Andrade called a potential “conspiracy to commit money laundering and wire fraud,” but ultimately determined that “the best avenue is probably a federal investigation because…these were Medicaid dollars.”  The diversion of Medicaid dollars requires immediate investigation. These are proceeds that rightfully belong to serve the citizens who rely on Medicaid, including children, pregnant women, neighbors with disabilities and those served by long-term care.

    Therefore, we respectfully urge you to investigate whether or not the $10 million settlement scheme violates federal law and complies with Centers for Medicare and Medicaid Services (CMS) legal and regulatory framework and any other applicable federal laws and regulations.

  • MIL-Evening Report: Banning young people from social media sounds like a silver bullet. Global evidence suggests otherwise

    Around 98% of Australian 15-year-olds use social media. Platforms such as TikTok, Snapchat and Instagram are where young people connect with friends and online communities, explore and express their identities, seek information, and find support for mental health struggles.

    However, the federal government, seeking to address concerns about young people’s mental health, has committed to ban under-16s from these platforms from later this year.

    There is no doubt social media presents risks to young people. These include cyberbullying, posts related to disordered eating or self-harm, hate speech, and the basic risk of spending long hours scrolling or “doomscrolling”.

    But is banning young people really the answer? We reviewed 70 reports from experts in Australia, the United Kingdom, the United States and Canada to understand what they recommend – and found broad agreement that a ban may not address the real problems.

    Humans preventing harm

    The overall verdict is that we need a much more thoughtful response than just a ban: only a coordinated approach between governments, regulators, tech companies and young people themselves will address youth mental health and online safety.

    We should be asking what we can do to make online spaces safer for young people, not jumping straight to removing them entirely.

    Content moderation is one area in need of urgent attention. Young people regularly report being exposed to harmful and age-inappropriate content on social media, while platforms replace moderation staff with cheaper AI systems.

    Automated processes have their place, but many recommendations in our review emphasised the importance of human moderators to keep up.

    Data and endless advertising

    A second issue exists around the collection and use of user data. Tech platforms have built their business model around user engagement and ad revenue.

    To keep users scrolling (and watching ads), companies collect large amounts of user data to deliver highly personalised feeds.

    Many experts advocate against the widespread collection and use of young people’s data, particularly for delivering advertising materials that promote dieting, unregulated supplements and cosmetic procedures. Posts like these often appear in an endless stream, interspersed between non-harmful and entertaining content.

    Starting with safety

    Alongside greater regulation of advertising material, many experts emphasised the need to consider “safety by design”.

    In other words, social media should be designed from the outset to prevent harming users. It may mean the end of “addictive” features such as infinite scrolling, frequent push notifications, and auto-play videos.

    Regulators also need the tools and power to hold platforms to account.

    That includes financial penalties, more transparent reporting from big tech companies, and taking proactive steps to keep harmful material off these platforms – not just taking down content after the fact.

    Age-checking tech troubles

    Our review did find a small number of reports that recommend barring young people from social media. However, experts questioned the feasibility of age verification technology and raised privacy concerns.

    The federal government has passed the buck to social media companies for actually implementing age verification of users.

    Platforms must take “reasonable steps” to restrict access by under-16s. It is unclear what these steps will be, but the prospect of facial recognition or digital ID checks raises serious privacy concerns.

    Others argue that banning under-16s from social media will drive them to less regulated online spaces, including online forums such as the notorious 4Chan, where some pages have an explicit “no rules” policy.

    It is also important to acknowledge that many young people find important support and communities on social media. Taking away social media may present risks to mental health in these circumstances.

    Listening to young people

    An age ban sounds decisive but comes with its own set of questions.

    In the absence of social media, where do young people questioning their sexual or gender identity go to find information and support? What would a ban mean for young people who engage with news on social media?

    There is little evidence about what impact a ban will have on young people, particularly those from diverse backgrounds.

    What’s more, young people have had minimal input into the policy. They have the insight to offer practical, real-world insights into what works and what does not.

    A blanket ban does nothing to make social media platforms safer for users. It might just delay problems and expose young people to an avalanche of harm when they log on at the age of 16.

    A ban brings its own risks

    The push to ban social media for under-16s is driven by genuine concerns. But unless it is a part of a broader, more thoughtful approach to online safety, it risks doing more harm than good.

    If we want a healthier digital environment, we can’t just lock out young people and hope for the best.

  • MIL-Evening Report: How accurate are my medical records? You might be surprised how often errors creep in

    Medical records of hundreds of patients at a Sydney hospital’s cancer genetics service have been reviewed following irregularities related to care by a single specialist.

    According to St Vincent’s Hospital, in about 520 records, there were matters such as poor documentation, incomplete correspondence and a lack of genetic counselling.

    In about 20 records, there were errors that carried potential risk – even if, ultimately, there had been no harm to patients – such as providing incorrect information and advice.

    Every now and again, cases like these make the headlines. Some examples of flawed medical records relate to individual human error. Some relate to issues in how electronic patient record systems are designed.

    These and other reasons mean errors can arise when records are created, accessed and shared.

    A huge potential for errors

    Health-care records describe the symptoms, conditions or problems being treated. They contain details about a patient’s medication, diet, mobility, social history, family concerns, observations, test results and language spoken. Health-care workers also document the plan to restore health, and progress. So entries must be correct, complete and timely.

    However, the scale of health-related communication and documentation is vast.

    Each day on average in Australia there are more than 33,000 hospitalisations, more than 112,000 out-patient services provided and more than 24,000 visits to emergency departments.

    Every interaction with a health-care professional requires notes to be made in a medical record.

    For example, a patient in a metropolitan public hospital is likely to be seen by at least three teams of nurses in a day, two or more junior doctors or registrars, as well as a specialist. Physiotherapists, speech therapists and other allied health workers may also be involved in someone’s care. Health-care teams record notes on paper, in electronic health records or a combination.

    There are also the millions of medical records updated in general practice, or by allied health workers outside hospitals.

    What type of errors are common?

    Accurate and timely medical records are supposed to allow staff to make safe clinical decisions, and to provide high-quality and continuous care. However, errors have been discovered in several audits and studies, including those related to medications.

    One review looked at how adverse drug reactions were recorded in electronic health records at one large Australian hospital. It found half of the reactions recorded lacked the minimum information required to inform clinicians about future treatment. One-third of records misclassified the type of reaction.

    A study of medication charts in Australia and New Zealand found at least one simple error on the medication charts of about 94% of the records reviewed. These included illegible drug names, missing information and inadequate documentation of allergies.

    One study from the United States found written errors, such as unclear documentation or not using plain language, were among the most common communication errors in the records analysed.

    What happens when there are errors?

    Errors in health-care records can spread, affecting how health-care professionals communicate with each other about the patient, potentially affecting care.

    Missing or inaccurate records can affect evidence collected as part of criminal, coronial or medical negligence investigations.

    As some hospital funding relies on the number and types of patients and interventions recorded, inaccurate records can affect health budgets.

    With inaccurate records, national and international collection of correct health-care information can be compromised.

    What causes errors?

    Errors in health-care records are caused by missing or incomplete information, including when health-care workers do not document changes.

    Difficulty in quickly finding important information, or delays in reporting new information, can contribute to errors, misdiagnosis and inappropriate treatment. This could be due to the ease of use of the electronic health record, the bulky or disorganised paper record or that health workers are busy.

    Health-care teams report using a mixed record systems (using both paper and electronic records) can cause problems.

    Then there’s “note bloat”, when staff copy and paste information from one place to another. This allows wrong information to perpetuate. This is a well-known hazard leading to errors, stress and wasted time.

    Abbreviations used in health-care records, particularly in medication charts, can be misunderstood or misinterpreted.

    An Australian study found one in three medication errors were technology-related and due to poor design or functionality.

    A Swedish study involved patients reviewing notes in their own medical records. It found almost 36% of patients found an error and more than 26% found an omission. About 18% of patients were offended by the content of the notes.

    What can we do?

    Improving the accuracy of medical records is not just health workers’ responsibility, although clearly they have a major role to play. Their workplaces, the IT companies that design the electronic systems, even patients, can also play a role.

    Health workers can make sure medical records are complete, accessible, accurate, readable and long-lasting.

    Workplaces, such as hospitals, can highlight in training and education the importance of documentation and how poor practices can lead to errors, and contribute to safety and quality problems.

    IT companies can design electronic health records that support how health workers need to communicate with each other, and the way they work.

    Patients can ask their health provider to correct errors found in their records, including in My Health Record.
