  • MIL-OSI Asia-Pac: Healthcare system set for reform

    Chief Executive John Lee announced in his Policy Address that the Government will conduct a comprehensive review on the positioning and objectives of the healthcare system.

    “The review will cover the following areas: reforming the functions and division of work among the Hospital Authority (HA), the Department of Health and the Primary Healthcare Commission, strengthening health promotion and disease prevention in primary healthcare, and improving public healthcare services.

    “In parallel, we will reform private healthcare services in terms of their quality, cost effectiveness and price transparency.”

    Advancing primary healthcare development

    Elaborating on the measures, Mr Lee said primary healthcare development will be carried out in 10 ways:

    (1) formulating legislation to strengthen the regulatory framework of primary healthcare and authorise the Primary Healthcare Commission to set up quality assurance and monitoring mechanisms;

    (2) developing a community drug formulary and launching a community pharmacy programme to help the public obtain affordable, primary-healthcare drugs;

    (3) devising health promotion strategies by adopting a life course framework to formulate health management plans for the public according to age and health conditions;

    (4) revamping maternal and child health and family planning services to strengthen pre-pregnancy counselling and parental education and promote healthy fertility;

    (5) strengthening the Whole School Health Programme to recommend targeted school-based measures for physical activities, meals and other matters to improve students’ physical and psychological well-being;

    (6) upgrading more District Health Centre Expresses into District Health Centres, and expanding the service network, and integrating the services of Woman Health Centres and Elderly Health Centres;

    (7) expanding the Chronic Disease Co-Care Pilot Scheme to cover blood lipid testing; positioning the HA’s general out-patient services as the comprehensive, primary healthcare service providers for the underprivileged;

    (8) formulating risk-based screening programmes for prevalent cancers, and implementing hepatitis B screening to prevent liver cancer;

    (9) launching a Primary Dental Co-Care Pilot Scheme for Adolescents to encourage the prevention of dental diseases, as well as rolling out a Community Dental Support Programme to enhance dental services for underprivileged groups; and

    (10) continuing efforts in tobacco control.

    Enhancing healthcare services

    In his Policy Address, Mr Lee highlighted that the Government will strengthen the HA’s public healthcare services by the following means:

    (1) reviewing the structure and levels of the HA’s fees and charges to encourage prudent use of services and direct resources to patients who need them most, while increasing the support for patients with financial difficulties and strengthening the financial sustainability of the targeted subsidisation of public healthcare services;

    (2) strengthening the centralised procurement of drugs and medical devices by various clusters of the HA system in order to enhance their bargaining power and expedite the introduction of new drugs;

    (3) formulating a directory for inherited and rare diseases by using the Hong Kong Genome Institute’s genomic data, while supporting research and clinical trials to promote precision medicine;

    (4) fully integrating the paediatric services of various clusters at Hong Kong Children’s Hospital and developing more advanced healthcare services to make the best use of the Children’s Hospital;

    (5) finalising the projects and timetable of the Second Hospital Development Plan to dovetail with the development of the Northern Metropolis and address local districts’ needs;

    (6) setting up the first stroke centre and the second chest pain centre;

    (7) enhancing the triage system and referral arrangements for specialist out-patient services, including setting up inter-specialty, integrated, out-patient clinics to avoid the need for multiple referrals; and

    (8) increasing the service capacity for cataract surgeries by at least 20%.

    As regards the quality and efficiency of healthcare services, the Chief Executive said that the Government will establish a professional platform for developing evidence-based clinical protocols and explore the feasibility of devising service quality and efficiency standards for public and private healthcare sectors.

    In addition, quality indicators will be developed for public and private healthcare systems.

    To enhance service efficiency and address the issue of medical inflation, the Government will explore legislating for private healthcare price transparency.

    Furthermore, the Government will seek amending relevant legislations to require all healthcare providers to deposit essential health data in the personal eHealth accounts of citizens, so that people can have more complete electronic health records and better continuity of medical care.

    Bringing in healthcare professionals

    The Government will promote the use of the legislation passed earlier to proactively admit more non-local doctors, nurses and dentists to enhance manpower, Mr Lee said, adding that a bill on the admission of qualified non-locally trained supplementary medical professionals will be introduced next year.

    Establishing third medical school

    The Chief Executive pointed out in the Policy Address that the Government supports the plan by local universities to establish a third medical school in Hong Kong.

    “A task group will be set up, inviting universities interested in establishing the new medical school to submit proposals. The Government will set aside sites in the Northern Metropolis Ngau Tam Mei to develop the new medical school campus and build an integrated medical teaching and research hospital.”

    Promoting development of Chinese medicine

    The Government will publish the Chinese Medicine Development Blueprint next year to take forward measures that helps Hong Kong develop into a bridgehead for the internationalisation of Chinese medicine (CM), Mr Lee said.

    One of the measures relates to exploring the application of big data to foster international research collaboration on herb-drug interaction to discover more evidence of clinical significance, promoting the internationalisation of CM.

    The blueprint will also promote the expansion of integrated Chinese-Western medicine services to cover more diseases in which CM has an advantage, including respiratory diseases and knee osteoarthritis, and to progressively extend the cancer care programme to all hospital clusters.

    Furthermore, the first Chinese Medicine Hospital and the permanent premises of the Government Chinese Medicines Testing Institute are expected to be completed and begin phased operation next year, while the first edition of the Hong Kong Chinese Medicine Cultural Festival will be held, Mr Lee added.

    Promoting mental health

    The Chief Executive also outlined measures, based on a medical-educational-social collaboration model, to promote mental health.

    The Government will develop a stepped care model for mental health, which comprises a multi-disciplinary framework with tiers, from dealing with general emotional problems in the frontline to handling cases requiring follow up and more serious mental illnesses cases.

    “The framework sets out the roles of different professionals and their division of work in the provision of mental health services for cases in each tier, enabling them to work together and perform their respective roles smoothly,” Mr Lee explained.

    An annual promotional theme will be set for the Mental Health Workplace Charter, and recognition will be given to participating organisations for achieving targets. The 4Rs Mental Health Charter in schools will also be campaigned to promote the mental health of students, teaching staff and parents in a more holistic manner.

    The Government will extend and enhance the Three-Tier School-based Emergency Mechanism, and launch the Mental Health Literacy resource packages for senior secondary and lower primary levels. A real-time, online youth emotional support platform will be set up in the second quarter of next year.

    A Transitional Support Service Teams for Persons in Mental Recovery will be set up, offering support to discharged patients waitlisted for halfway house service. The Social Welfare Department will also launch an additional Integrated Community Centre for Mental Wellness.

    The Government will also strengthen teachers’ capacity in the early identification of, and support for, students with mental health needs, and assist parents in acquiring the knowledge and skills in addressing children’s mental health.

    “ICDRA is crucial in sharing knowledge, building partnerships, and working in harmonisation to ensure safe and effective medicines for everyone. How well are we doing in regulation – our efforts can lead to better health outcomes for people all over the world.” This was stated by Smt. Anupriya Singh Patel, Union Minister of State for Health and Family Welfare during her address at the 19th International Conference of Drug Regulatory Authorities (ICDRA), here today. Dr VK Paul, Member (Health), NITI Aayog was also present.

    The event which is being hosted for the first time in India, from 14th – 18th October by the Central Drugs Standard Control Organization (CDSCO), Ministry of Health and Family Welfare, in collaboration with the World Health Organization (WHO) brought together regulatory authorities, policymakers, and health officials from over 200 countries.

    Addressing the session, Smt. Patel emphasized on the new rules and regulatory procedures introduced in India. She said, “new regulations published in the areas of clinical trial as New Drugs and Clinical Trial rules 2019 and Medical Device Rules 2017 have promoted scientific and ethical research at par with global expectation and international practices. The medical device rules include risk-based classification, bringing all devices under regulation through registration and framing regulatory pathway.” She further stated that “there is robust pre-approval and post approval regulatory procedures of all medical devices, diagnostics managing product lifecycle indicating robust control.  We are collaborating globally with international organizations such as IMDRF, ISO, WHO and regional network like SEARN to harmonise regulatory requirements in the area of medical devices and diagnostics.”

    The Union Minister noted that India has been recently recognised as an affiliate member of IMDRF. “Recognition of Indian Pharmacopoeia by the Pharmacopoeial Discussion Group (PDG) is another milestone marking the harmonisation and recognition of regulatory standards”, she added.

    Smt. Patel highlighted that the recently published revised Schedule M, aligned with WHO requirements of good manufacturing practices for various products including biologicals, investigational products, further establish the harmonisation goal. “This coupled with e-governance for all regulatory procedures have established good regulatory practices in the regulation of medical products in India”. She also informed that “AMR containment is another priority area in which India is building its strategy for effective management and control.”

    Lauding WHO’s efforts in upgrading the regulatory systems of different countries, Smt. Patel stated that “the way WHO promotes partnership, reliance, experiential learning, pharmacovigilance systems, anticounterfeiting technologies and monitoring systems and reduction in use of animal experimentation need a word of appreciation. What is more important is the model of collaboration, deliberation and inclusive decision making which bring all the stakeholders together for best outcome.” On this note, Smt. Patel also underscored India’s commitment to work closely with WHO in contributing to many of these areas. “We have wealth of knowledge and appreciation for inclusive processes. Hosting ICDRA is a demonstration of our intent and commitment towards Global Public Health”, she said.

    On the Government’s commitment to Healthcare, the Union Minister said that “the focus is on building a strong healthcare system that meets the needs of our population. Programs like Ayushman Bharat ensure that over 500 million people have access to quality healthcare. This shows our commitment to making healthcare a right for everyone, not just a privilege.”

    She also highlighted the Union Government’s enthusiasm about how Artificial Intelligence (AI) is changing the face of healthcare. “AI can assist us in making quicker decisions, enhancing patient care, and speeding up research. By adopting new technologies, we can offer improved services to our people and achieve better health results”, she said.

    The Union Minister concluded her address by urging everyone to work together for a healthier future. “The ICDRA is not just a conference; it is a chance for us to collaborate, innovate, and support one another in our shared mission for better health for all”, she stated.

    Speaking on the occasion, Dr VK Paul said that “quality medicines improve the quality of life, human productivity as well as the way of life.” He said that this year’s ICDRA is crucial for its commitment to strengthen regulatory environment worldwide, especially coming after the Covid-19 pandemic.

     

    Dr Paul highlighted the efforts being taken on healthcare sector by the present government such as the launch of the world’s largest health assurance scheme and a massive effort being taken in digital health. He said the future of healthcare in India and the world will be driven by technology and noted that India with its rich talent pool, strong government initiatives on digital health and medical infrastructure can be a driver of this change.

    Dr Paul informed that India licenced 8 vaccines for use during the pandemic through a proper regulatory process. He noted that India developed different variants of vaccines including mRNA, DNA, nasal vaccines etc which were available for a fraction of the cost of other vaccines that were available in the world.

    He also pointed out India’s heritage of a rich traditional system of medicines being practiced for hundreds of years. He underscored the importance of mainstreaming such traditional medicinal practices which can aid in improving the healthcare of people.

    Dr Rajiv Bahl, Secretary, Dept of Health Research and DG, ICMR highlighted the crucial role of regulators in health research. He said, “In the first three months of the pandemic, India developed indigenous tests at one-fortieth of the cost. Similarly, within nine months of the pandemic, India approved a Covid-19 vaccine.” He informed that three diagnostic tests have also been developed for MPox which were approved by CDSCO.

    Dr Yukiko Nakatini, Assistant Director-General, WHO noted that ICDRA 2024 is the first ICDRA after the Covid-19 pandemic. She noted that one urgent need highlighted by the pandemic was the need for a strong regulatory system. Dr Yukiko also congratulated India for its achievement of retaining Maturity level III for vaccine regulation.”

    Ms. Kimberlee Trzeciak, Deputy Commissioner, US Food and Drug Administration, USA highlighted the opportunities and risks brought about by the introduction of advanced drug manufacturing practices as opposed to the traditional methods. She also emphasized on the need to ensure compliance with quality and underscored the importance of collaboration between drug regulatory bodies across the world.

    Himalayan Mountaineering Institute establishes Darjeeling’s first-ever Sewage Treatment Plant; Can treat 1,000 litres of waste water per day

    The Department of Defence as part of the Pan-India Swachhata Abhiyan initiative, has successfully covered 2,705 sites out of 3,832 locations, creating positive impacts across the country. As on October 15, 2024, over 20,976 physical files have been reviewed, leading to the weeding out of 5,391 files and freeing up 195k square feet of valuable space. Revenue generation of Rs 21.1 lakh has been achieved through the disposal of scrap materials and obsolete IT equipment.

    The sites include those under Military Hospitals, Controller General of Defence Accounts, Border Roads Organisation, Indian Coast Guard National Cadet Corps, Sainik Schools, Canteen Stores Department, Cantonments along with the Nehru Institute of Mountaineering, Uttarkashi and Himalayan Mountaineering Institute, Darjeeling.

    The cantonments have been at the forefront of the campaign and carried out initiatives like mosquito-breeding eradication campaigns and organised waste segregation workshops for local communities in coordination with volunteers. Garbage Vulnerable Points (GVPs) have been transformed into plantation sites, further enhancing public spaces with dry-leaf composting initiatives in parks.

    Himalayan Mountaineering Institute established Darjeeling’s first-ever Sewage Treatment Plant (STP) named Swachhata Se Samriddhi which can treat 1,000 litres of wastewater per day, equivalent to 365 kilo litres annually. The treated water is repurposed for toilet flush systems, ensuring sustainable waste management within the institute. Additionally, the Institute has constructed a Rain Water Storage Plant with a capacity of 1.8 lakh litres, significantly reducing reliance on external water sources.

    The Himalayan Mountaineering Institute also exemplifies the ethos of sustainability by transforming damaged mountaineering gear, such as shoes and ropes, into decorative pieces, highlighting the innovative spirit of recycling and environmental stewardship.

    The National Human Rights Commission (NHRC), India is organizing a function to celebrate its 31st Foundation Day at Vigyan Bhawan on the 18th of October 2024. On the occasion, the Chief Guest, Shri Jagdeep Dhankhar, the Vice-President of India will address several national and international dignitaries in the presence of NHRC, India Acting Chairperson, Smt Vijaya Bharathi Sayani and Secretary General, Shri Bharat Lal and other senior officers of the Commission. The Foundation Day celebrations are a reminder of the Commission’s commitment to protecting and promoting human rights.

    Following this, the Commission will also be organizing a day-long national conference on the ‘Rights of Older Persons’ under the theme- ‘Assessing the Structural Framework, Legal Safeguards, Security Rights, and Institutional Protection for India’s Elderly.’ The conference will address various concerns of older persons under three key technical sessions including ‘Addressing the Aging Population,’ ‘The Gendered Perspective of Aging,’ and ‘Evaluating the Healthcare Landscape-Impact on Healthy Living, Productivity, and Social Security.’ These sessions will be attended and addressed by various stakeholders including eminent experts and civil society representatives.

    The NHRC, India, recognizes senior citizens as valuable assets to society. It is essential to honour their contributions to nation-building by promoting their overall welfare, respecting their rights, and encouraging meaningful engagement. The Commission has a core group on the Rights of older persons to discuss and suggest measures for their welfare. Recently, the Commission assessed the institutional responses and support available to them. It also issued an advisory to ensure the rights of elderly persons during Covid-19. Besides promoting awareness about the need to protect the rights of the elderly in the country, the NHRC has been emphasizing the proper implementation of policies and laws including the Maintenance and Welfare of Parents and Senior Citizens Act, 2007 (MWPSC Act, 2007).

    In addition to the rights of older persons, the Commission has been working to promote and protect the rights of all segments of society, particularly those who belong to the vulnerable sections. During the 31 years of its journey since inception on 12thOctober, 1993 to 30thSeptember, 2024, the Commission has handled 2305194 (23 lakh 5 thousand and 194) cases including 2,873 cases of suo motu cognizance and recommended the payment of monetary relief of more than Rs. 254 crore in 8,731 cases to the victims of human rights violations.

    During the last one year w.e.f. 1st October, 2023 to 30th September, 2024, the Commission disposed of 68,867 cases and recommended more than Rs. 17.88 crore as monetary relief in 404 cases to the victims of human rights violations. It also registered 112 cases taking suo motu cognizance during this period. Besides, 19 spot inquiries were conducted into the allegations of human rights violations.

    The NHRC, India has conducted numerous spot investigations, open hearings, and camp sittings since its inception. Reviews of innumerable bills and laws, conferences and research projects, 31 Advisories, as well as more than 100 publications, including monthly newsletters, thousands of media reports, and engagements in international forums bear testimony to the work of the Commission towards the promotion and protection of human rights.

    The 31 advisories issued by the Commission including the recent, among others are Child Sexual Abuse Material (CSAM), Rights of the Widows, people involved in begging, the Right to Food, Right to Health and mental Health, Rights of the Informal Workers, Upholding the Dignity of the Dead, Rights of Truck Drivers, Environmental Pollution and Degradation, Advisory for ensuring the welfare of Transgender Persons, Advisory to mitigate Deliberate Self Harm and suicide attempts by prisoners and Advisory to Prevent, Minimize and Mitigate Ocular Trauma.

    The NHRC, India has designated 14 Special Rapporteurs to assess human rights conditions across various regions of the country. They conduct visits to shelter homes, prisons, observation homes, and similar institutions, compiling reports for the Commission that detail their observations and suggestions for future action. Additionally, the Commission has also appointed 21 Special Monitors tasked with overseeing specific thematic human rights issues and reporting their findings to the Commission. Throughout the year, they have visited several places to suggest improvements in human rights situations.

    The Commission has also proactively engaged with the NGOs and Human Rights Defenders. It has constituted 12 core groups on various thematic issues related to human rights to hold discussions with the domain experts and the concerned senior government functionaries representing different ministries from time to time to finalize its recommendations for the government. Besides these core group meetings, the Commission also organizes open house discussions with different stakeholders on various issues of human rights. The Commission during the last one year, w.e.f. 1st October, 2024 to 30th September, 2024 has organized 13 core group meetings and 06 open house discussions on varied themes of human rights and two national consultations.

    The NHRC, India is actively caring for all 47 Government mental health hospitals across the country. It continues to collaborate with central and state governments, parastatal organizations, academic institutions, NGOs, and human rights defenders to protect and promote human rights for all. Since last year, the Commission started a new program of sensitizing All India Services officers, including IAS, IPS, and IFS officers. The goal is to equip officers with a deeper understanding of human rights, enabling them to share this knowledge within their respective organizations for imparting human rights training to the other personnel.

    The Commission has also collaborated with various institutions to conduct human rights awareness programmes. During the last one year w.e.f. 1st October, 2023 to 30th September, 2024, it organized 69 collaborative workshops and 08 moot court competitions with financial support of over Rs. 130 lakh to various institutions. Additionally, the Commission also organized on-site winter and summer internships and 06 online short-term internships which have benefitted hundreds of students from far-flung areas at zero costs on their travel expenses to help them evolve as human rights ambassadors. During this period, students and faculty from 45 institutions visited the Commission to learn about various aspects of human rights and the functioning of the NHRC. Besides, annual debate competitions for Central Para-military forces and State Police organizations on various aspects of human rights for sensitization of security personnel.

    The Commission has issued notices to various sports bodies to establish Cells to deal with cases of harassment of women in the workplace. It has been issuing regular directions to provide free housing to thousands of homeless persons as per the government scheme. Victims of communal riots and internal conflicts are compensated. The Commission constantly endeavours to rehabilitate persons displaced due to natural disasters, land acquisition, and other causes. In the cases of suicide by debt-ridden farmers, the Commission successfully intervened.

    Some of the other important interventions of the Commission include recommending amendments to 97 laws that discriminate against persons with Hansen’s disease. The Government has enhanced compensation for bonded labour based on NHRC advisory at the pre-trial stage.

    The Commission has also been playing an active role at international human rights forums including the Asia Pacific Forum of National Human Rights Institutions, Global Alliance of National Human Rights Institutions (GANHRI), and UN Human Rights Council, among others with the participation of the Chairperson, Members, and senior officers. Last month, it successfully hosted a two-day conference of the NHRIs of Asia Pacific.

    Twelve Thematic Core Groups involving various experts have helped the Commission to formulate mechanisms to evaluate the schemes initiated by the Govt. and make recommendations thereof. The Special Monitors and Special Rapporteurs who are the eyes and ears of the Commission are adding value to the mandate of the Commission.

    The Commission has taken several new initiatives to expand its outreach including linking its HRCNet Portal with all the State authorities and the majority of State Human Rights Commissions. Any person can file complaints directly through online mode in a fast and efficient manner and can track the real-time status of their complaint on the Commission’s portal. The online complaint filing system is also linked with over five lakh Common Service Centers and the National Government Services Portal.

    Olivia Cruz, a budget analyst lead assigned to the command’s Resource Management and Acquisitions Directorate (DRMA), is a representation of the highly professional and diverse civilian workforce within Navy Medicine.

    A 2014 graduate of Texas A&M University at San Antonio, Cruz directly supports the allocation and execution of all Research, Development, Test, and Evaluation (RDT&E) and Operation and Maintenance (O&M) funding received by NAMRU San Antonio. This includes performing or advising on work in any of the phases or systems of budget administration of funds required for command programs. Additionally, she serves as the command’s Time and Attendance clerk.

    A San Antonio native, Cruz began her Navy civilian career in 2021 with Naval Medical Forces Development Command serving as the regional labor and budget analyst.

    “At my previous command, I was the only person on board who was born and raised in San Antonio,” said Cruz. “Most of the times, the comptroller would refer newly arriving personnel to me for places in the city to visit and eat. Eventually, I joked with telling people that I was the unofficial San Antonio Chamber of Commerce.”

    According to Cruz, who has been maintaining and executing government budgets for 15 years, there are countless opportunities to serve locally, out of state and even overseas within Navy Medicine.

    “Working for NAMRU San Antonio has provide me with insight on the numerous opportunities available for my son in science research as he begins his journey in biology,” said Cruz, whose goal is to complete her career at the federal level. “When I initially came on board with the command, I didn’t realize that I would be working directly with some of the Navy’s smartest research scientists.”

    “As analysts, we don’t see the type of research that is being conducted. Usually, all we see is the money side,” added Cruz. “Is there funding? Are the funds committed? The list goes on, but we never see the scientists at work.”

    Cruz feels that being able to observe some of the work being done at NAMRU San Antonio enables her to understand how important her role is in the command.

    “Being able to know how my job directly supports research has motivated me to be a more knowledgeable analyst which allows our scientists to efficiently research projects that will essentially save countless lives,” said Cruz. “Our DRMA team doesn’t allow setbacks and limited staff to stop us from hindering our scientists from their research mission.”

    Perseverance is one of Cruz’s strengths.

    “I have always persevered; however, working for the Navy has instilled in me the motivations to keep going even if I feel that I am not performing to my personal standards,” said Cruz. “However, I have the great opportunity of working with so many diverse personalities and professionals from whom I continue to learn from. They have shown me that there is more for me to learn and how to overcome challenges.”

    Continuing to listen, learn, and apply shared knowledge from others has enhanced Cruz’s importance of her role as an analyst with NAMRU San Antonio. In the future, she wants to share her knowledge with others to guide and help make them better in their profession.

  • MIL-OSI United Kingdom: Eplontersen approved to treat adults with rare inherited nerve disease or polyneuropathy

    The Medicines and Healthcare products Regulatory Agency (MHRA) has approved eplontersen (Wainzua) to treat adults with polyneuropathy associated with hereditary transthyretin amyloidosis (ATTRv), which is a rare progressive condition that runs in families caused by a protein called transthyretin (TTR).

    In people with this disease, small fibres of TTR protein clump together to make deposits called ‘amyloid’. Amyloid can build up around or within the nerves, heart and other organs, stopping them from working properly.

    Eplontersen is specifically indicated for use when the disease is causing polyneuropathy, which is damage to multiple nerves outside of the brain and central nervous system, resulting in pain, discomfort, progressive weakness and loss of sensation in the legs and arms, and mobility difficulties.

    Eplontersen works mainly by lowering the amount of TTR protein made by the liver. As a result, there is less TTR protein in the blood to form amyloid deposits. That can help reduce the effects of the disease.

    The recommended dose of eplontersen is one 45mg dose every month, administered as an injection under the skin using a pre-filled pen. Treatment with eplontersen lowers the amount of vitamin A in the blood and patients will need to take vitamin A supplements during treatment.

    The MHRA’s approval of the medicine is supported by evidence from a NEURO-TTRansform study. In this trial 168 adult patients with hereditary transthyretin-mediated amyloidosis received a subcutaneous injection of eplontersen 45 mg every 4 weeks compared to an historical placebo arm from the older NEURO-TTR study completed in 2017 as well as patients who received another medicine called inotersen 284 mg weekly.

    The study looked at levels of TTR in the blood of patients to measure the levels of this protein that contributes to the disease. It also used questionnaires to rate the changes that patients reported in the symptoms of their nerve damage. This study tracked these changes from the start of the trial (baseline) to 35 and 66 weeks after the patients had received eplontersen.

    Patients in the trial receiving eplontersen experienced greater reductions in TTR levels and less worsening of the disease from baseline compared to the placebo group.

    Niaxo brings a wealth of experience in data platforms and analytics, with its founders having clocked up “something ridiculous” like 60 years’ worth of contracting into government as independent consultants between them.

    They saw a niche, with experience of both working independently within government as well as working with government clients, to do something different and get technical projects off the ground faster – avoiding the so-called analysis paralysis which can afflict larger organisations.

    And so Niaxo was born in September 2019.

    The team is now significantly larger, with the initial team joined by ten staff, but retains a core focus on bring novel techniques to other people’s data problems.

    An area the company is increasingly excited about, says Ed Puddicombe, Niaxo’s director of strategy and business engagement, is using artificial intelligence (AI) and synthetic data to generate simulated scenarios. These could be used for interviews, for example, or for training materials in areas such as law enforcement.

    For one Accelerated Capability Environment (ACE) customer, the company used large language models to find examples of the types of text it wanted to replicate synthetically and then designed a wrapper to make an initial version of a template, which has been well received as a starting point. Technology and applications such as this could be a game-changer in terms of speeding up project timelines.

    Puddicombe said: “It was a project for ACE that was the genesis of starting to do some really cool stuff with synthetic data and we enjoy being part of interesting work and working on novel challenges.”

    The Niaxo team heard about ACE from contacts and, intrigued, set up a meeting to find out more. They signed up, and completed their first project in early 2020, exploring data-driven decision-making at a key point of the criminal justice system.

    Puddicombe said: “We took an initial data-processing idea, and within six weeks revamped it into more of an enterprise tool.”

    One of the major ACE commissions Niaxo has been part of was the covid response for the Joint Biosecurity Centre, which was set up to provide evidence-based insight to aid local and national decision-making.

    The company designed and built the data science platform which calculated the ‘r’ value – the measure of how quickly infections were growing across the UK. A rough design on a piece of paper evolved into a platform where every data output could be reversed, to understand the data feeding into it.

    Niaxo built a demonstrator in five days, had the first user live in five weeks, and within five months was told it was the biggest data science platform in government, with hundreds of data scientists working on it. The company also ran and continued to develop it for a number of months, before it was migrated “overnight, seamlessly” to the Department of Health and Social Care.

    Another project Niaxo worked on for a law enforcement body was around understanding the mathematical principles of how online grooming in chats could be detected, and how pattern matching could help, no matter which language was being used.

    Puddicombe added: “I think working with ACE illuminates problem spaces we may have overlooked, so we’ve got a skill that we didn’t know we could grow, from a seed that wouldn’t otherwise have been sown.”

    Providing humanitarian support for civilians affected by the current conflict was the focus of the Minister for the Middle East’s, Hamish Falconer, first official visit to the region this week.

    Announcing a new package of support to assist civilians medically evacuated from Gaza to Egypt, the Minister pledged £1 million of UK assistance to the Egyptian Ministry of Health that will support medically evacuated civilians from Gaza who are receiving care in Egypt.

    Delivered through the World Health Organisation (WHO) in Egypt, the funding will provide vital medical supplies and medications, including chemotherapy and rehabilitative equipment, to those in need. It will also strengthen capacity to care for patients from Gaza with chronic diseases.

    Minister for the Middle East Hamish Falconer said:

    “As a key regional partner, Egypt plays a central role alongside the UK in working for stability and security across the Middle East and driving forward de-escalation efforts in both Gaza and Lebanon.  

    “The worsening humanitarian situation in Gaza continues to bring devastation to many lives, with many requiring life-saving support over the border in Egypt. That’s why, alongside our Egyptian partners, we are funding life-saving treatments and support for medically evacuated civilians from Gaza. 

    “At Al Arish, I saw that many tonnes of lifesaving aid continue to be denied entry into Gaza by Israel. As winter approaches, Israel can and must do more to ensure aid flows freely into Gaza and to facilitate the UN and humanitarian agencies to carry out their work safely.

    “I signed a new UK-Egypt Memorandum of Understanding on sustainable food security, launching a new partnership between our two countries to tackle a critical global challenge.”

    The announcement came during the Minister’s visit to Al-Arish, where he met with the North Sinai Governor to discuss aid flows into Gaza. Here, the Minister visited the Egyptian Red Crescent’s warehouse and Al-Arish General Hospital to see how UK funding to WHO Egypt will be used.

    The Minister also met with Egypt’s Foreign Minister, Badr Abdelatty where he thanked Egypt for its ongoing role in getting aid into Gaza and agreed the need to continue working together for an immediate ceasefire in Gaza and Lebanon. Minister Falconer also raised the consular case of Alaa Abd El-Fattah and called for quick progress on his release.

    As Egypt is a regional leader in ensuring sustainable food security for the Middle East and Africa, the Minister also agreed a landmark UK-Egypt Memorandum of Understanding on Food Security with Rania Al Mashat, Minister of International Cooperation and Alaa Farouk, Minister of Agriculture. The agreement includes technical assistance to advance sustainable agribusiness practice and increase crop yield in Egypt.

    Speaking on today’s funding announcement, World Health Organisation Representative to Egypt Dr Nima Abid said:

    “WHO values its long-standing partnership with the British government, and we are deeply grateful for this generous contribution from the Foreign, Commonwealth and Development Office.

    “This support will enable WHO, in collaboration with the Ministry of Health and Population, to deliver critical supplies to Egyptian hospitals and equip healthcare workers to meet the urgent needs of medical evacuees from Gaza. I would also like to express my appreciation for the government of Egypt for its vital role in treating patients from Gaza and in facilitating their safe evacuation to other countries as well.”

    The UK continues to play a leading role in alleviating the suffering in Gaza and continues to provide significant funding to partners to support those most in need. The UK has already provided 78,000 shelter items, 76,000 wound care kits, and 1.3 million items of medicine.

    The UK trebled its aid commitment to the OPTs in the last financial year and this Government will maintain significant funding this financial year to support trusted aid
     agencies on the ground.

    The UK continues to call for an immediate ceasefire on all fronts and continues to push for urgent aid to enter Gaza and reach those most in need.

  • MIL-OSI China: 6 killed, 43 injured in Israeli airstrike on municipal building in S. Lebanon

    Six people, including the mayor, were killed, and 43 others wounded on Wednesday in an Israeli airstrike targeting the buildings of the Nabatieh Municipality in southern Lebanon, according to Lebanese medical and military sources.

    The sources, speaking on condition of anonymity, told Xinhua that an Israeli warplane fired four air-to-ground missiles at the Nabatieh Municipality building and the building of the Union of Municipalities, killing the mayor, Ahmad Kahil, and a number of municipality members, injuring several others, and destroying the infrastructure.

    The Civil Defense teams, the Lebanese Red Cross, and the Islamic Health Authority are working to remove the rubble of the two buildings and search for missing persons, the sources said.

    Lebanese caretaker Prime Minister Najib Mikati condemned the attack, saying it deliberately targeted a meeting of the municipal council discussing the city’s service and relief situation.

    The new “aggression” conveys the message that “the international community should act immediately instead of remaining silent about Israeli crimes,” Mikati said.

    Meanwhile, Lebanese Ministry of Foreign Affairs and Emigrants announced on Wednesday that it had filed a new complaint with the UN Security Council regarding the Israeli attacks on Lebanon during the period Oct. 3-14.

    This is part of the periodic complaints submitted by the ministry through the Permanent Mission of Lebanon to the UN headquarters in New York to document the Israeli offensive and pressure the international community and the Security Council to act to stop it.

    Since Sept. 23, the Israeli army has been conducting intensive airstrikes on Lebanon in a sharp escalation with Hezbollah.

    The death toll from Israeli airstrikes on Lebanon since the beginning of the Hezbollah-Israeli conflict on Oct. 8, 2023 has reached 2,350, while injuries went up to 10,906, Health Ministry figures showed on Tuesday.

  • MIL-OSI United Kingdom: Mental Health Partnership goes from strength to strength

    A Sellafield Ltd investment in The West Cumbria Mental Health Partnership via our Social Impact, Multiplied programme is making a difference to our community.

    Since our initial investment of £1.8 million in 2022, the partnership which is delivered by Cumbria Community Foundation has:

    • provided funding to more than 20 charitable organisations to provide accessible, community-based mental health support services
    • provided support to more than 5,000 adults and 2,000 young children and young people
    • of those supported in the first three years, 61% reported improved mental health or wellbeing, 58% reported increased self-esteem and confidence, and 58% saw a reduction in stress and anxiety

    Kelvyn James is an international mountain leader, qualified counsellor and volunteer with the Samaritans.

    He founded the social enterprise Mental Health North West which secured £31,320 of funding via the partnership to deliver 120 guided walks over 3 years for people experiencing mental health issues.

    Participants are not required to have had a formal diagnosis. They join the walks if they feel it would help them. There is no charge.

    Kelvyn said:

    It’s a spectrum, from those who are suicidal to people experiencing a bad day. When we take them for a walk, it’s an opportunity to talk.

    We have one chap who says the walks are the only things in his life that he looks forward to. We have a lady who says it’s the only time that she speaks to other people.

    The partnership has 3 core activity areas: support to adults, support to children and young people, and to maintain recovery via the Recovery College.

    More than 80 organisations who now meet regularly to understand the services available, how they can refer people to those services, and any gaps in support.

    Confirming that our financial support to the West Cumbria Mental Health Partnership will continue in this financial year, our head of corporate sustainability and supply chain development, Eirini Etoimou, said:

    Continuing to empower the West Cumbria Mental Health Partnership is a testament to our commitment to fostering resilience and well-being in our communities.

    With the support of our Social Impact, Multiplied programme, and the dedication of the Cumbria Community Foundation, we are paving the way for innovative mental health initiatives that will uplift lives and strengthen the fabric of West Cumbria.

The U.S. Department of Health and Human Services CED program expands employment for low-income communities facing persistent poverty and high unemployment by investing in community development corporations.

    Sunderland Care and Support (SCAS), one of the largest social care providers in the region, will hold a recruitment fayre on Thursday 24 October at the Leechmere Centre, where attendees will be able to find out more about working in care, meet staff and senior managers, and even get support with applying for roles.

    Councillor Kelly Chequer, Sunderland City Council’s Cabinet member for Health, Wellbeing and Safer Communities, said: “As a leading provider of adult care services for some of the most vulnerable individuals in our community, SCAS operates a diverse range of vital services designed to support those with conditions ranging from autism to mental health challenges to complex needs.

  • MIL-OSI United Kingdom: Event celebrates the Tak£500+ project success within local communities!

    The recent Tak£500+ Participatory Budgeting Project exceeded all expectations with an incredible £105,906 allocated to 108 fantastic local community projects – and an event to celebrate all of the wonderful projects took place at Seagoe Hotel, Portadown.

    Over 200 people came along to hear how the Tak£500+ Project had benefited communities right across the borough and to hear more about Participatory Budgeting and how it enables local people to decide how public funds are used to address needs in their area.

    Organised by the Armagh, Banbridge and Craigavon Community Planning Partnership, an amazing 4286 people voted for the projects they wanted to see receive up to £1,000 funding, and the impact these projects left on each area was remarkable. All projects were centred around the Take 5 public health message.

    “This was the third year of the Tak£500+ project and it really did exceed all expectations once again,” commented the Lord Mayor, Councillor Sarah Duffy, “and the success of this project is all down to the local communities who put forward their ideas and then made them a reality.

    “Putting the decision making process in the hands of local people to tell us how public funds can be spent to address needs in their areas is vitally important and ensures that this money is spent in the best way possible, to really make a difference and a lasting impact on our residents

    “We are lucky to have such a rich community life in this borough and along with the Community Planning Partnership – and in particular the PB Working Group – made this project a huge success with lasting results across the borough.”

    Colette Rogers, Chair of Community Planning Strategic Partnership added, “This event was a wonderful reflection on just how meaningful and valuable the Tak£500+ Project is and demonstrated how powerful local communities can be at making decisions that leave a real and lasting impact in their areas.

  • MIL-OSI USA: S. 3022, IHS Workforce Parity Act of 2024

    S. 3022 would modify two workforce development programs aimed at recruiting health professionals for the Indian Health Service (IHS). The IHS Scholarship Program provides grants to current students who are members of federally recognized tribes and working toward degrees in the health professions. The awards cover tuition and education-related expenses in exchange for a two-year, full-time commitment to work for IHS after certification as a health professional. The Loan Repayment Program pays current health professionals up to $25,000 annually to cover student loan repayments and up to $6,000 a year to cover the associated income tax liability in exchange for a two-year, full-time commitment to work for IHS. 

    S. 3022 would modify both programs by allowing recipients to work part-time for twice the number of years as full-time recipients in exchange for the assistance. The aim is to improve recruitment of health professionals at IHS, which has reported chronic medical staff shortages for many years.

    Manitoba Government is Prepared for Respiratory Virus Season


    The Manitoba Government is prepared for the respiratory virus season with a comprehensive strategy designed to reduce the strain on the health-care system and keep Manitobans healthy, safe and informed, Health Seniors and Long-Term Care Minister Uzoma Asagwara announced today.

    “I encourage every Manitoban to get vaccinated and keep yourself, your loved ones and your broader community safe,” said Asagwara. “The previous government failed to prepare for respiratory virus season, they made cuts to health care and refused to listen to front-line workers. We’ve made significant progress in one year and the system is prepared to meet an increased need for care during this difficult season.”

    Preparation includes:

    • building up intensive care unit (ICU) and acute care beds so that everyone gets the care they need:
    • the Manitoba government has newly funded 18 ICU beds, for a total of 110 adult ICU beds, and eight pediatric ICU (PICU) beds, plus four step-down beds, for a total of 21 PICU beds; and
    • the Manitoba government has opened 112 acute care beds with a plan to open 70 more, 43 transitional care unit beds and 27 more opening in the coming months.  
    • vaccines to protect Manitobans and their loved ones:
    • influenza (flu) and COVID-19 vaccines are available at medical clinics, ACCESS centres, vaccine clinics, pharmacies, nursing stations and through public health;
    • the Manitoba government is providing the RSV vaccine free of charge to seniors over the age of 60, living in a long-term care home, and encouraging residents to get vaccinated through their long-term care facility; and
    • the Manitoba government launched a provincewide ad campaign to inform Manitobans about respiratory virus season and encourage everyone to get vaccinated as soon as they are able.

    The minister noted the government knows staffing is critical when it comes to beds and the Manitoba government hired 873 net new health-care workers to the system in six months, marking over halfway to its goal of hiring 1,000 healthcare workers this year.

    “Considerable planning has occurred within the health system in an effort to mitigate or minimize service disruptions during respiratory virus season,” said Dr. Jose Francois, chief medical officer, Shared Health. “This planning has been strongly supported by our partners in government, who have worked in collaboration with clinical leaders to ensure Manitoba’s health system is as fully prepared as it possibly can be for the coming respiratory virus season.”

  • MIL-OSI USA: Tonko, Turner Launch Inquiry to Ensure Access to Addiction Treatment for Opioid Use Disorder

    ALBANY, NY — Representatives Paul D. Tonko (D-NY) and Mike Turner (R-OH) today announced that they have sent inquires to dozens of hospitals, insurance plans, pharmacies, pharmacists, and providers aimed at gathering information on their existing actions and plans to eliminate barriers for accessing medications for opioid use disorder (MOUD).

    The letters are intended to gain insight into the successes and barriers that have arisen following the implementation of the Congressmembers’ law, the Mainstreaming Addiction Treatment (MAT) Act. That bill, which was signed into law in early 2023, aims to expand access to the proven addiction treatment buprenorphine by increasing the number of medical professionals who can prescribe buprenorphine for opioid use disorder from 130,000 to 1.8 million. With this barrier eliminated, work continues to implement the MAT Act to the fullest extent and have addiction treatment accessible in every community. This goal will require many communities, providers and interests to step up and lead.

    “For the millions of Americans battling substance use disorder, having access to proven, affordable treatment can mean the difference between life and death,” Congressman Tonko said. “I worked tirelessly to pass the Mainstreaming Addiction Treatment (MAT) Act into law to open the doors of access to the proven addiction treatment, buprenorphine. We must use every tool possible to realize the dream of the MAT Act. I look forward to hearing the responses and insights from these healthcare stakeholders about how we can best move forward to ensure that those on the path to recovery have the treatment they need and deserve.”

    “Passing the MAT Act into law last Congress was a crucial step forward in our country’s battle against opioid addiction and substance use disorder,” said Congressman Turner. “Too many Ohio families have been ripped apart by the opioid epidemic that has swept over the nation. The MAT Act offers greater access to life-saving care to those struggling with addiction, and we are already seeing positive results as drug overdose deaths dropped by 10% in 2023. However, our work is not finished. We must ensure this progress continues, and it is my hope that these healthcare providers can offer greater insight for Congress to combat opioid addiction and help save lives.”

    As co-chair of the Addiction, Treatment, and Recovery Caucus, Tonko has long spearheaded federal efforts to address the overdose epidemic and expand addiction treatment. He, along with Rep. Turner also champion the Reentry Act, bipartisan legislation that empowers states to restore access to healthcare, including addiction and mental health treatment, through Medicaid for incarcerated individuals up to 30 days before their release.

    Most recently, Tonko introduced the BUPE for Recovery Act, legislation that would require the Administrator of the Drug Enforcement Administration (DEA) to temporarily exempt buprenorphine from the Suspicious Orders Report System (SORS) for the remainder of the opioid public health emergency.

    Today, the Honourable François-Philippe Champagne, Minister of Innovation, Science and Industry, and the Honourable Mark Holland, Minister of Health, made the following statement:

    “As scientists and researchers work to solve some of the world’s most pressing challenges, their work becomes increasingly complex and interdisciplinary. To keep pace with this evolution, Canada must ensure its research support system also evolves and becomes more collaborative to best meet the needs of our diverse science and research communities. That is why in Budget 2024, our government announced that it will establish a new capstone research funding organization, within which the granting councils—the Natural Sciences and Engineering Research Council of Canada, the Social Sciences and Humanities Research Council of Canada, and the Canadian Institutes of Health Research—will continue to exist and support excellence in investigator-driven research, including a clear and direct linkage with the Health Portfolio. This new organization will allow Canada to leverage the best parts of the current system, while also modernizing it to bring more coordination, cohesion and agility.

    “In June 2024, we asked the federal granting councils to jointly undertake engagement with the research community to help inform the government’s ongoing work to create the new capstone organization. We would like to thank the granting councils and all who participated in the engagement for their valuable input that will help shape the future of the Canadian science and research support system.

    “The granting councils have jointly submitted to us their summary report: What We Heard: Tri-agency engagement with the research community on modernization of the federal research support system. The government has reviewed the report and is taking it under consideration to advance the design and implementation of the capstone organization and a truly modernized, collaborative, inclusive and responsive research support system. We will continue to seek the perspectives of the science and research community, including Indigenous partners, researchers and communities, to ensure the strength of the new organization.

    Quick facts

    • Since 2016, including initiatives proposed in Budget 2024, the federal government has invested over $22 billion in science and research initiatives, including infrastructure and emerging talent, as well as other science and technology support measures.
    • Budget 2024’s investments include providing $825 million over five years, and $199.8 million per year ongoing, to increase support for master’s and doctoral students and post-doctoral fellows, as well as $1.8 billion over five years, and $748.3 million per year ongoing, to the federal granting councils to increase core research grant funding and support Canadian researchers.
    • The government’s work to modernize the research ecosystem is informed by the findings of the independent Advisory Panel on the Federal Research Support System.

    “Canada is deeply disturbed by the recent events in the Middle East. The violence must stop, and a diplomatic solution must be found before further human tragedy ensues.

    “We urgently call for a ceasefire and the immediate release of hostages.  

    “Canada condemns the Israel Defence Forces (IDF) increasing attacks on civilian infrastructure in northern Gaza as well as its attacks on the United Nations Interim Force in Lebanon [UNIFIL] positions along the Blue line. These are unacceptable and must immediately stop. 

    “The latest attacks on Al-Aqsa Hospital, the school in the Nuseirat camp and a food distribution centre have caused the deaths of dozens of civilians seeking refuge, including women and children.

    “The increasingly dire humanitarian situation is unacceptable and continues to deteriorate due to a significant decrease of aid allowed into Gaza. An increase in humanitarian aid is desperately needed to end this suffering.

    “The Palestinian civilian population has been displaced countless times, with nowhere safe to go and is unable to meet its most basic needs. As this conflict escalates, it is civilians who continue to bear the brunt of the violence and suffer through its lasting consequences.

    “We reiterate our call for an immediate ceasefire which is desperately needed to end this suffering in Gaza. We continue to strongly condemn Hamas’ terrorist attacks against Israel.

    “Canada also condemns Hezbollah’s continued attacks on Israel. Hezbollah’s ongoing and unprovoked attacks on Israel serve only to further destabilize the region.

    “Canada calls for an immediate ceasefire between Israel and Hezbollah in Lebanon. We condemn the killing of civilians in Lebanon, Gaza, the West Bank and Israel. We continue to insist that civilians be protected and not be inflicted with damage.

    “We call on all parties to the conflict to uphold their obligations under international law to ensure the safety and security of civilians, as well as the protection of first responders and UNIFIL personnel, at all times. 

    “Canada continues to support calls for a ceasefire and for a political solution to be found through diplomatic efforts.

    “We need a comprehensive, just and lasting peace for the Israeli, Lebanese and Palestinian peoples.”

    Katherine Martinez, it’s great to see you again. You are certainly a Veteran “worth knowing,” and I’m glad you could join us today. And thanks very much for your powerful remarks, especially about how VA can fit into Veterans lives, rather than Veterans trying to fit their lives into VA.

    Jaime Areizaga-Soto and our panel members—thank you for the great conversation this morning.

    And thank you, Nathan Maenle and team, for putting together the program today.

    I don’t have to look far to see Hispanic Americans making an impact at VA—serving Vets as well as they’ve served all of us.

    To close today’s program, I’d like to tell you about a few extraordinary VA teammates. One demonstrating leadership in a moment of crisis. Another, an example of a Veteran dedicating his life to serving other Vets. A third, a young cemetery director committed to honoring our fallen Vets and preserving their stories. And finally, an emerging leader dedicated to healing human suffering wherever he finds it.

    First, Fernando Rivera. Fernando’s the Executive Director of the Southeast Louisiana Health Care system and was born in Cuba. His mother was a government public relations professional. His father was a military officer. Both were disillusioned—to put it mildly—with the oppression of Castro’s communist regime, and secretly participated in the movement to overthrow the government. In 1961, Fernando’s father was imprisoned in Havana and paid the ultimate price for his efforts to ensure his family and country could live in freedom. He was executed.

    Fernando immigrated to the United States—New Orleans—in 1969 as a political refugee. He was nine years old. His mother worked three jobs to support him and his grandparents. As a young man he started at VA as a GS-5 Project Engineer—and advanced to roles including Medical Center Director, Network Director, and Acting Deputy Under Secretary. He’s been with VA for 39 years and counting. Along the way, in 1992, he met the love of his life—his wife Stacie—at VA. That’s part of the reason Fernando calls VA his family.

    Ten years ago, he went home—so to speak—when he helped bring VA healthcare back to New Orleans nearly a decade after the devastation caused by Hurricane Katrina. Fernando’s leadership during the COVID-19 pandemic set national standards, caring for over 1,600 infected patients, and coordinating responses across VA and private-sector healthcare systems. Just last month, Fernando’s steady leadership guided the Veterans, staff, and volunteers at VA when Hurricane Francine made landfall in Louisiana. Fernando’s team responded quickly. They kept in close contact with the most vulnerable patients. When the storm had passed, Fernando had the medical center and 6 of 7 community-based outpatient clinics up and running within 24 hours and the final, most impacted clinic, up within 48 hours.

    Every day when Fernando goes to work, he reads a sign at the front of the hospital in New Orleans. It says, “The price of freedom can be seen within these walls.” Fernando says that he can never take for granted those who have served in uniform because they are the reason he and his family are free. Perhaps few can appreciate that freedom more profoundly than Fernando.

    Next, I’d like to talk about Francisco Vazquez. Francisco is the Medical Center Director at the Houston VA. His story of service starts with his beloved father, also named Francisco. His father grew up an orphan in Puerto Rico and saw the Army as a way out of the cycle of poverty and hopelessness he experienced. He joined the famous 65th Infantry Regiment shortly after his 18th birthday in 1951. Within a year, he found himself landing on the shores of Inchon, Korea. He stayed in the Army and Francisco, the younger, was born in Fort Chafee, Arkansas. When Francisco was eight, his father retired from the Army and moved the family back to Puerto Rico where he experienced firsthand the richness and warmth of the family-centered culture there.

    Francisco is very proud of his Puerto Rican heritage—where, as he says, “the island is beautiful … the food is amazing … and the people are welcoming, valiant, and determined.” After graduating from college, Francisco attended Navy Officer Candidate School, following his father in service to the nation. He served on the USS Okinawa for four years. His VA journey began in 1995 at the Central Texas Healthcare System. Since then, Francisco has served in seven VA hospitals. “We have the best mission in the world,” Francisco says. “We help our nation pay back part of the debt of gratitude we owe our Veterans for their service and sacrifice through the delivery of world-class care.”

    And for Francisco, it all goes back to the values his father modeled for him and his siblings during his Army career: honesty, hard work, humility, and putting the needs of others above your own. It was these values that inspired Francisco’s oldest son, Armando, to join the Army. And though Francisco’s father and son are no longer with us, their memory inspires and motivates him every single day to fulfill the second part of what President Biden calls our one truly sacred obligation as Americans, to prepare and equip the troops we send into harm’s way, and then care for them and their families when they come home. Three generations of Hispanic Americans who have put service and love of country at the forefront of their lives and are an example for us all.

    Next, I’d like to tell you about Marcos De Jesus. Marcos is the Director of the South Florida National Cemetery in Lake Worth, Florida. He was born in the Dominican Republic. When he was four, his mother—Maria—and he immigrated to Puerto Rico where he says he truly found his home. Marcos’ commitment to service started with the values taught by his mother—the belief that even if you don’t have much, you should still strive to give back to the world. By the time Marcos left for college it was only natural that he should join the ROTC program there, where the Air Force’s core value of “Service Before Self” resonated with him. Marcos served five years as an Air Force officer, deploying to Colombia and Abu Dhabi. He was impressed with the fact that in the Air Force he met people from all walks of life who were dedicated to one thing: serving their country.

    Like many Vets, when Marcos left the military, he searched for a similar sense of purpose and service. He found that purpose in the National Cemetery Administration in 2021.  His life came full circle when he returned to Puerto Rico for NCA’s Cemetery Director’s Development Program. Marcos takes pride in learning and sharing Veteran stories preserved in our National Cemeteries, especially through the Veterans Legacy Memorial site. Along the way, he met and married his wife, Barbara, from Caracas, Venezuela. Being a part of a Venezuelan family now, Marcos sees similarities in the values he witnessed growing up in Puerto Rico: courage, determination, and perseverance. Marcos jokes that he knows he and his wife will have debates about which heritage their children will claim. But they are certain of one thing—their children will inherit their shared values of hard work and service.

    Finally, I’d like to tell you about Dr. Alexander Tenorio. Alexander is a White House Fellow—one of America’s most prestigious programs for emerging leaders—serving with us here at VACO. Hispanic alumni of this program include the Honorable Carlos Del Toro, Secretary of the Navy, the Honorable Jaime Areizaga-Soto, our Chairman of the Board of Veterans Appeals, and Alberto Ramos, my Chief Speechwriter. Alexander and his family immigrated to the United States from Mexico in the 1980s, fleeing violence in their hometown. Alexander grew up poor in South Los Angeles, selling clothing on the streets to help his family make ends meet. It was on those streets that he saw drug use, gang violence, and the healthcare challenges the people in his community faced.

    So, he decided to go to medical school and become a neurosurgeon, one of the most under-represented specialties. During his residency in San Diego, Alexander came face-to-face with the horrific reality that is all too common along our southern border—individuals sustaining life changing wounds like Traumatic Brain Injury—T.B.I.—and spinal cord damage after falling from the 30-foot border wall—fleeing the same violence his parents did in the ‘80s. He’s dedicated his life to raising awareness of these tragic and preventable accidents, advocating for increased access to care for Hispanic and non-citizen populations. Alexander says that some of the most grateful patients he cared for as a resident in San Diego were our Vets. And that’s part of his motivation to serve his fellowship here at VA. His parents, who sacrificed so much to help him become a neurosurgeon, instilled in him the core value he lives by today: to help the helpless, and to heal human suffering wherever he finds it.

    So, why did I tell you about these extraordinary individuals this morning? Well, because frankly, these folks—Katherine, Francisco, Fernando, Marcos, and Alexander—know better than most what America is all about. And we need more leaders like them at VA. Because a diverse VA is a stronger VA. We all travel our unique journeys in life. Whether from Cuba, Puerto Rico, the Dominican Republic, Mexico, or Minnesota. And the convergence of those varied paths is what makes us stronger. We all cherish our freedoms as Americans.     A study published in JAMA looks at the use of Levonorgestrel-Releasing Intrauterine Systems and breast cancer risk. 

    Dr Channa Jayasena, Reader in Reproductive Endocrinology, Imperial College London, said:

    The Levonorgestrel-Releasing Intrauterine Systems (LNG-IUS) is a highly effective form of contraceptive for women. Unlike contraceptive pills, LNG-IUS releases a progesterone-like hormone directly into the womb.  This means that levels of the hormone are much lower than when you take it as a tablet. It is well known that prolonged use of the contraceptive pill slightly increases breast cancer risk. However, we have always assumed that the LNG-IUS would not increase breast cancer risk due to the much lower levels of hormone exposure to the whole body. The results of this study are therefore highly unexpected. A large population of women taking LNG-IUS was compared with a similar sized population of women not taking any contraceptive medication. While the researchers tried to make sure that the two populations of women were matched for things like age, weight and education, other behaviours like smoking have not been measured. So, women not taking contraceptive medication may have been healthier in other ways compared to the LNG-IUS group.

    The study found that there were 14 extra cases of breast cancer per 10,000 women in the group using LNG-IUS compared to those not using contraceptive medication. It is difficult to tell how this compares with taking the contraceptive pill which has been studied in different populations. It is unfortunate that this study did not compare breast cancer risk between the LNG-IUS and oral contraceptive pill. But it is important that smoking, alcohol and obesity are much more important risk factors for breast cancer than contraceptive medications.

    We always need to be vigilant to new health risks from medical treatments, and this is no exception. My advice for women is that breast cancer risk caused by LNG-IUS is not established but warrants a closer look.”

    Dr Mangesh Thorat, Honorary Reader in Wolfson Institute of Population Health, Queen Mary University of London and Consultant Breast Surgeon, Homerton University Hospital, Queen Mary University of London (QMUL), said:

    “This new large Danish study adds to the existing literature evaluating the risk of breast cancer in women using hormonal forms of contraception. The study specifically investigated the use of hormonal intrauterine devices, commonly known as hormonal coils, for example – Mirena coil. Similar to the British study (Fitzpatrick and colleagues) published last year, this study shows a small increase in the risk of breast cancer associated with the use of such coils. The existing evidence suggests that the increase in the risk is similar to that with oral contraceptive use. It is worth noting that this association has been known for at least a few years and a broad consensus exists among the medical fraternity that the overall benefits of hormonal contraception outweigh the harms like such small increase in the risk of developing breast cancer. Furthermore, as the baseline risk of breast cancer is lower in younger individuals, any increase in the risk associated with the use of hormonal coil is also smaller. The risk increases with increasing age and it is reasonable for women near 40 years of age to have a discussion with their healthcare practitioner regarding non-hormonal modes of contraception.”

    Rule change has caused financial challenges for Children’s hospitals serving defense communities, limited their ability to provide care

    WASHINGTON – Today, U.S. Senators John Hickenlooper and Michael Bennet and U.S. Representatives Doug Lamborn and Jason Crow, along with 16 of their Senate and House colleagues, sent a letter to Secretary of Defense Lloyd Austin urging the Defense Health Agency (DHA) to address the financial burden caused by a change in the way children’s hospitals are reimbursed for the care they provide to military families covered by TRICARE, the government health care program for active duty service members and their families.

    “We write to express our deep concerns about a 2023 Defense Health Agency (DHA) rule that catalyzed a major shift in the TRICARE reimbursement methodology for children’s hospitals,” wrote the lawmakers. “Children’s hospitals situated in defense communities in our home states are now grappling with the impacts of this change.”

    DHA previously exempted children’s hospitals from the adult Medicare reimbursement process because the program’s policies aren’t applicable to the care children typically need. Over 2.4 million children obtain care from children’s hospitals through TRICARE each year, and the change has placed an outsized burden on children’s hospitals in major defense communities, like Colorado Springs. Specifically, the Children’s Hospital Colorado said one in five patients in their Colorado Springs facility pay with TRICARE. The rule change is expected to cost them over $25 million annually.

    The Children’s Hospital Association (CHA) sent letters to the Department of Defense in 2020 and 2023 expressing their concerns about the proposed rule. However, they did not receive a response before the DHA implemented the change in October 2023.

    The lawmakers specifically asked the following questions:

    • What dialogue has DHA had with the affected children’s hospitals to understand how this new reimbursement methodology impacts operations and access to care?
    • What data and sources informed the agency’s analysis of the impact on children’s hospitals that care for TRICARE patients?
    • How did the agency account for the financial impacts of military families traveling for care in circumstances where local services are no longer available?
    • How did the agency develop the contingency payment and why did the DHA set a lower contingency payment for pediatrics?
    • Can the agency verify the number of children’s hospitals that are expected to qualify for the contingency payment that is outlined in the rule?

    Hickenlooper has publicly supported reversing the rule change and offered an amendment to the annual National Defense Authorization Act that would have defrayed some of these costs.

    On October 15, SPbPU opened the VI International Forum “Advanced Digital and Manufacturing Technologies”. The key topic of the forum is the development and application of advanced digital and manufacturing technologies as the basis for Russia’s technological leadership. In recent years, we have seen how almost the same notes of our lectures with the terminology of advanced digital and production technologies are approved in regulatory documents, consolidating the scientific and technological groundwork formed by the ecosystem of technological development of SPbPU over many years.

    Thus, the speaker noted the adoption of the national standard GOST R 57700.37-2021 “Computer models and modeling. DIGITAL DOUBLES OF PRODUCTS. General provisions” in Russia and in the international arena.

    In 2023, the global digital twin market was valued at $10 billion, and by 2028, experts estimate it will reach $110 billion with an unprecedented annual CAGR growth of 61%. World leaders recognize digital twins as one of the technologies of the future, the speaker explained.

    Director of Science at Gazprom Neft PJSC, Honorary Doctor of SPbPU Mars Khasanov presented an expert opinion on the implementation of system digital engineering technology, including digital twin technology, and considered the possibilities of combining it with neurosymbolic artificial intelligence to solve the company’s problems. Mars Magnavievich emphasized the need for practice-oriented training of personnel and highlighted various formats of project interaction at Gazprom Neft PJSC to form the required set of competencies of a future specialist.

    At the plenary session, representatives of research centers, leading universities and industrial enterprises exchanged experience in the application of new technologies, assessed the dynamics of their development and the speed of implementation in real production practice, held a discussion on the main trends in the development of domestic engineering software and discussed current issues in engineering education.   • MIL-OSI USA: Rep. Lawler Announces Over $32 Million in Federal Funding in FY25, Set to Deliver More Than $70 Million For District in First Term

    Yesterday, Congressman Mike Lawler was joined by Rockland County Sheriff Lou Falco and other local elected officials outside the Rockland County Sheriff’s office to highlight major infrastructure and public safety investments for the Hudson Valley secured in federal community project funding for Fiscal Year 2025. Although the appropriations work for Fiscal Year 2025 is still ongoing, Congressman Lawler has already secured more than $32.9 million for fifteen different projects across New York’s 17th Congressional District. This is in addition to $38 million in federal funding for 17 projects in Fiscal Year 2024. With the latest numbers, Congressman Lawler is set to deliver more than $70 million for New York’s 17th Congressional District in his first term in office.

    “When I was elected to represent this district, I promised I would work with my partners at every level of government and in both parties to prioritize the needs of our residents,” said Congressman Lawler. “That’s exactly what I’ve done in working with each of the officials with me here today to ensure funding for each of these important projects.”

    “Projects like these are great examples of what we can accomplish when we put aside politics and focus on meeting the needs of our communities and constituents,” Congressman Lawler concluded. “That’s been my focus as a Congressman, it’s been my focus throughout the appropriations process for both Fiscal Year 2024 and 2025, and it will always be my focus.”

    Congressman Lawler is one of the most bipartisan members of the 118th Congress and represents New York’s 17th Congressional District, which is just north of New York City and contains all or parts of Rockland, Putnam, Dutchess, and Westchester Counties.

    Below is a list and description of each project announced by Congressman Lawler:

    Rockland County Public Safety and Crime Prevention Initiative

    Recipient: Rockland County Sheriff’s Office

    Amount: $4,500,000

    Summary: The funding will be used for necessary law enforcement priorities such as police vehicles, equipment to be utilized at a new reality-based public safety training facility, and the structural analysis and design of the facility. This project will provide necessary support to the Rockland County Sheriff’s Office to ensure effective enforcement of laws, address crimes, and improve the criminal justice system across the County. These priorities closely align with the purposes of the Byrne Justice Grants program.

    Law Enforcement Reality-Based Training Facility

    Recipient: Rockland County Sheriff’s Office

    Amount: $2,600,000

    Summary: The funding will be used to construct a new reality-based training facility for the Rockland County Sheriff Department to grow law enforcement capacity, ensure effective law enforcement operations, and overall enhance public safety throughout the county. This project will increase law enforcement capacity and enhance development of public safety in Rockland County, NY. This new facility will allow the Sheriff’s office to better combat crime, improve community safety, and foster an environment more conducive to economic development.

    Westchester Pleasantville Water District Storage Tank Replacement/Rehabilitation

    Amount: $2,250,000

    Summary: The funding will be used to mitigate risks attached to their leaking North and South Water Storage Tanks. Both 1-million-gallon water storage tanks are housed within the Millwood Water Treatment Plant approximately 3 miles north of the Village and services the entire Pleasantville Water District and approximately 2,000 residents in the Town of Mount Pleasant. Over the course of their 90-year history, both storage tanks have been exposed to natural deterioration, which has led to their useful life cycles coming to an end. Funding for the proposed Water Tank Improvement project will replace both of the Village’s North and South water tanks, which will significantly reduce the tax burden placed on residents. The completion of this project will positively impact the Pleasantville water distribution system, which services all 7,513 residents (U.S. Census Bureau’s 2020 Decennial Census) and an additional 2,000+ residents living in the Town of Mount Pleasant. Without funding, Pleasantville’s North and South Water Tanks would continue to suffer from leakage which will compromise the Village’s water supply, further increasing distribution risks that will negatively affect residential quality of life.

    Nanuet TOD Project

    Recipient: Town of Clarkstown

    Amount: $5,000,000

    Summary: The funding will be used for construction of the first phase of its transformative vision for the Nanuet Transit Oriented Development Plan (TOD), including water and sewer infrastructure improvements, road, sidewalk, and pedestrian safety and streetscape enhancements. Bringing this extensive new infrastructure will spark the redevelopment of the area by mitigating the drainage issues plaguing development in the area, enhancing the appeal of the area, and creating the necessary connectivity for a walkable and easily accessible TOD neighborhood. This transformational project will lead to an expansion of workforce housing, parking, and economic opportunity for the community.

    Safer Ramapo Project

    Recipient: Town of Ramapo

    Amount: $5,000,000

    Summary: The funding will be used for installation of pedestrian safety measures, including traffic signals, turning lanes, walking paths, and sidewalks in the Town of Ramapo. Directing funding towards pedestrian safety measures will not only mitigate the risk of accidents but also cultivate a more pedestrian-friendly environment. This will improve accessibility and safety, improve connectedness, and improve ADA compliance. The Town of Ramapo is also home to one of the largest Orthodox and Hasidic Jewish populations in the country, with many of these individuals walking to shul on Saturdays. To ensure the safety of these individuals, the Town of Ramapo requires additional funding for the construction of sidewalks. From 2011-2022, there have been 983 pedestrian injuries, and twenty-seven (27) deaths in the Town of Ramapo. Safer walking conditions continue to be an urgent need for Ramapo residents. 

    Carmel Water District II Water Plant Upgrade and Rehabilitation Project

    Recipient: Town of Carmel

    Amount: $1,250,000

    Summary: The funding will be used to repair/replace a 60-year-old facility and associated equipment to meet federal and state requirements of PFOAs. This project will directly benefit approximately 10,000 individuals on a daily basis. Beyond safeguarding public health, it will also have far reaching positive effects on various institutions and businesses in downtown Carmel, including the Putnam Hospital, and Carmel High School.

    Hallocks Mill Sewer District Extension Project

    Recipient: Town of Yorktown

    Amount: $1,250,000

    Summary: The funding will provide public sewer service to neighborhoods presently serviced by residential septic systems. The properties served by septic systems have generally been installed on small lots with steep slopes and environmental constraints, which are not ideal for septic system performance. The extended district will dramatically improve and enhance the water quality of Sparkle Lake, a major recreational resource for town residents, as well as the Croton Reservoir, which provides drinking water for millions of residents in the metropolitan area. The proposed wastewater infrastructure project, however, protects one of the largest public water systems in the United States. 

    Croton Falls Sewer Project

    Recipient: Town of North Salem

    Amount: $1,250,000

    Summary:  The project will install sewers in the Village of Croton Falls’ General Business district to replace existing septic and cesspools. The Village is struggling because some of its businesses, especially all of the restaurants which have mixed use with residential apartments over them, are on older septic tanks that cannot be upgraded due to a lack of space. Due to its proximity to the Muscoot Reservoir, many of the existing tanks and fields are on New York City Department of Environmental Protection (DEP) property and under roads, some are not fixable if they were to fail. To allow the Village to take advantage of its assets and begin to realize the potential of the community, Croton Falls needs sewers, which this funding will provide.

    Town of Orangetown Sidewalk Improvement Project

    Recipient: Town of Orangetown

    Amount: $1,500,000

    Summary: The funding would be used for streetscaping improvements in the Hamlet of Pearl River in the Town of Orangetown specifically to install sidewalks, curbs, and drainage improvements. This project provides streetscape improvements that meet a compelling local need consistent with the statutory purposes of the Economic Development Initiative. The corridor receiving these improvements has seen three fatalities in the last 15 years and is part of Orangetown’s continued efforts to improve pedestrian safety on Middletown Road. Sidewalks are a critical element of community well-being and serve as a connector between residents, neighborhoods, local economies, and more. Walkable sidewalks will ensure Orangetown is a place that attracts both businesses and new residents, which will surely promote economic development in the community.

    Village of New Square Road Infrastructure Improvements Project

    Recipient: Village of New Square

    Requested Amount: $4,000,000

    Summary: The funding will complete much needed improvements for NYS Route 45, widen Village roads, and complete paving improvements. The project will enhance traffic flow and increase the safety of motorists and pedestrians by addressing long-standing concerns about road conditions and motorist/pedestrian safety in the Village of New Square. Portions of the project area lack adequate sidewalks as a buffer from narrow, congested roads and are inherently dangerous in a fast-growing Village where many pedestrians utilize Village streets to travel to school, shopping, and visits to friends and family. By rectifying roadway deficiencies, the Village will relieve congestion, reduce travel time, and increase vehicle operating benefits.

    Memorial Drive Reconstruction Project

    Recipient: Village of Suffern

    Amount: $1,000,000

    Summary: The funding will be used for the reconstruction of Memorial Drive. The project will reopen a critical thoroughfare for emergency vehicles and for their timely response which can save lives, prevent injuries and protect property. Memorial Drive, which is currently closed due to the continual road deterioration and for the safety of residents, is also a major connection between the local community and US Route 202. Its reconstruction will stabilize the road for the safe use by all traffic including emergency vehicles.

    Downtown Water Main Lining Project

    Recipient: Village of Sleepy Hollow

    Amount: $983,200

    Summary: The funding will support a multi-phase project to address hydraulic inefficiencies in the downtown area, the Village of Sleepy Hollow will reline and replace approximately 1,980 linear feet of existing 8” water main line along a portion of Cortlandt Street and College Avenue. The current downtown water system is over 100 years old. The inner village neighborhoods bordering the downtown area represent the Village’s largest number of public housing units, low-income housing, low-income senior housing, and middle-income housing. This corridor is a critical aspect of public health and safety, connectivity and multi-modal transportation, affordable housing, resilient infrastructure, and overall economic investment. As a result, this project will provide immediate remediation and resiliency to infrastructure that otherwise poses a water quality and economic risk to the community, particularly its most vulnerable residents. 

    Samsondale Avenue Curbs & Sidewalk Replacement Project

    Recipient: Village of West Haverstraw

    Amount: $800,000

    Summary: The funding will be used to construct necessary upgrades of sidewalks and curbing along Samsondale Avenue, which have rapidly deteriorated and become a hazard. The project will unlock critical improvements to a main thoroughfare for a Village operating on a limited budget. Many pedestrians utilize Samsondale Avenue to get to the business district along Railroad Avenue and Route 9W as well as access to and from the Village of Haverstraw. The lack of sidewalks is potentially hazardous to people walking on them. This project will improve accessibility and safety, improve connectedness, and improve ADA compliance.

    Westchester County Public Safety Enhancements

    Recipient: City of Peekskill

    Amount: $520,000

    Summary: The funding will be used for expanding and modernizing the Peekskill police station, which is currently at capacity. The City of Peekskill’s police station is currently at capacity and urgently requires more space. With this necessary facility expansion and modernization, the City will be able to hire 10 more law enforcement officers, which will significantly improve operations.

    North State Road Water Main Replacement Initiative

    Recipient: Town of Ossining

    Amount: $1,000,000

    Summary: The funding will upgrade the community’s water distribution system on North State Road, an area that has a history of water main breaks, including 14 incidents over the last 5 years, that has exposed the general public to health risks and requires expensive repairs. It will improve the Town’s water distribution system as a part of a larger initiative to replace all aged cast iron water mains, significantly reducing the likelihood of water main breaks in a historically problematic area and positively impacting all 37,764 users in the service area.
